◇중이에 염증이 생기면?

 

귀는 겉에 보이는 귀바퀴와 귓구멍까지를 외이(바깥귀), 고막부터 달팽이관 뼈까지의 공간을 중이(중간 귀), 달팽이관보다 뇌에 가까운 안쪽을 내이(안쪽 귀)라고 부른다. 중이염은 공기로 채워진 고막 안쪽 중이 공간 내의 점막과 뼈에 생기는 염증이다.

 

중이 공간에는 망치 모양 혹은 종 모양의 작은 뼈가 있는데 이는 외부로부터 들어오는 소리에너지 때문에 생기는 고막의 떨림을 달팽이관으로 전달해주는 소리 증폭기 역할을 한다. 중이에 염증이 생기면 이런 소리 전달이 원활하지 않아서 소리가 작게 들린다. 표정을 만들어 주는 안면신경도 중이의 뼛속을 지나기 때문에 심한 중이염은 얼굴 한쪽이 움직이지 못하는 합병증이 발생할 수도 있다.

 

뿐만 아니라 안면신경은 단순히 눈이 안 감기고 입 모양이 삐뚤어지는 등 얼굴 근육 움직임에만 관여하는 것이 아니기 때문에 자칫 눈물, 침샘의 분비, 혀의 맛, 큰 소리로 인한 내이 손상을 차단하는 효과도 사라지게 된다. 중이 보다 더 안쪽에는 달팽이관 말고도 전정이라는 몸 균형을 잡아주는 센서가 있다. 따라서 심한 중이염이 깊은 속귀까지 침범하면 어지러움증이 심해진다.

 

◇ 증상별 중이염의 종류

 

귀는 밖으로 돌출되어 있지만 중요한 구조물은 귓구멍 안에 감추어져 있다. 이비인후과에 가면 현미경이나 내시경으로 고막을 확인할 수 있어서 중이염 여부를 쉽게 진단할 수 있다. 중이염은 고막 안쪽 점막의 염증 때문에 고막이 붉게 보이고 통증과 열을 수반한 급성 중이염, 중이 점막의 염증으로 인한 액체가 채워져 수영하다 귀에 물들어갔을 때처럼 먹먹하게 들리는 삼출성 중이염 그리고 고막에 구멍이 나고 누런 고름이 흘러나오며 청력이 떨어지는 만성중이염 등으로 구별할 수 있다.

 

급성 중이염은 증상과 눈으로 고막을 보는 것만으로도 진단이 되어 초기에 진단이 용이하고 어린이들에게서 흔히 생긴다. 항생제에 반응이 좋아 비교적 치료가 잘되는 편이다. 급성중이염이 있는 동안에는 고열이 날 수 있기 때문에 해열제가 필요하다.

 

반면에 삼출성 중이염은 고막에 채워진 염증액을 없애는 것이 치료이므로 약 한 달간의 투약으로 말려본다. 하지만 반응이 없으면 고막에 주사기로 물을 빼내거나, 고막 마취를 하고 고막을 째거나 환기관이라고 부르는 조그만 튜브를 고막에 박아 넣어 공기가 튜브를 통해 중이 점막에 직접 닿도록 하여 중이 점막을 말리는 수술을 하기도 한다.

 

중이염이 더 진행되고 오래 지속된 경우는 아무래도 급성중이염이나 삼출성 중이염 단계를 지나 누런 고름이 나오는 지경에 이르게 되므로 냄새도 나고 가렵기도 하고 잘 안 들리고, 혹은 이명이 심하게 느껴진다.

 

가장 진행된 형태의 중이염은 진주종성 중이염으로 일종의 종양성 질환이고 뼈를 녹이는 특성이 있다. 따라서 심한 합병증이 생기기도 하는데 안면마비, 어지럼, 심한 두통 등이 생길 수 있다.

 

2000년대 들어 한국의 의료, 위생 및 경제 관념이 좋아지면서 대표적 후진국형 염증성 질환인 만성 중이염의 빈도 자체가 감소하고 따라서 합병증도 급격히 줄어드는 추세인 것은 매우 반가운 일이다.

 

◇ 청력을 최대한 보존하는 내시경 귀수술

 

예전의 중이염 수술은 거대한 현미경으로 귀 뼈의 염증을 완벽하게 제거하는 것이 주요한 목표였다. 하지만 이제는 삶의 질 향상을 위해 청력을 최대한 보존하거나 개선하면서 최소한의 침습적 수술을 요구하는 경향이 짙어져서 최근에는 내시경 귀 수술이 주목을 받고 있다.

 

물론 비침습적이라는 것이 치료 기간도 짧고 더 간편해 보이기는 하지만 치료할 수 있는 범위로만 따지면 예전부터 해왔던 현미경 수술과 비교해 제한적인 것은 사실이다. 하지만 숙련된 의사에게 수술을 받는다면 요즘 이비인후과 외래에서 만날 수 있는 대부분의 귀 질환에서 적용이 가능할 정도로 눈부시게 발전하고 있다.

 

◇외래환자들이 궁금해 하는 중이염 수술

 

외래에서 많이 듣는 질문 중에 하나가 한 번의 중이염 수술로 귀에서 생기는 모든 증상이 없어지냐는 것이다. 물론 염증으로 인한 증상들은 상당 부분 개선될 것이다. 하지만 청력 신경은 재생되는 신경이 아니므로 오랜 염증으로 이미 변화가 왔을 가능성이 높고 수술로 염증을 제거한들 청력 회복은 제한적인 경우가 많다는 것을 이해해야 한다.

 

경우에 따라서는 중이 공간에 염증이 너무 심해서 염증은 제거하되 청력개선을 나중으로 미루기도 한다. 이런 경우는 2차 수술에서 청력을 개선한다. 수술하지 않고 있으면 안되냐는 질문도 있다. 특히 연세가 높거나, 만성질환으로 수술이라면 지긋지긋하신 분들이 수술에 대한 거부감이 심하다.

 

물론 요즘처럼 달나라에 가는 세상에서 좋은 선택은 아닌 것 같기는 하다. 하지만 어차피 선택은 환자의 몫이다. 최소한 합병증이 생길 우려가 있는 환자라면 의사가 극구 수술을 권할 것이지만, 그래도 수술을 선택하지 않을 수는 있다.

 

하지만 그냥 내버려 두지는 말자. 그렇다고 항생제를 콩알 먹듯 할 수는 없는 터, 좋은 관리 방법을 추천한다. 당장 동네 수퍼마켓에 가서 식용 사과식초를 한 병 사고 약국에서 생리식염수를 한 통 사서 두 액체를 1:1로 섞어서 귓구멍에 적당량 넣어 씻어 내자. 이른바 ‘귀세척’이다. 이런 방법으로 염증을 줄이는 건데, 간혹 통증을 호소하는 사람이 있다.

 

그런 경우라면 1:1이 아니고 식초1에 생리식염수 2를 넣거나 3을 넣어 사용하자. 그리고 혹시 귀세척을 하는 동안에 어지럼이 발생할 수 있으니 희석액은 체온과 같은 36도 정도로 맞춰서 사용하자. 실제로 이 방법은 많은 대형병원에서 귀 수술 이후에 수술 부위 안정을 위해 흔히 쓰는 방법으로 큰 해가 없는 좋은 자가 치료 방법이다. 다만 누군가의 도움으로 귀세척을 한다면, 훨씬 간편한 치료법으로 요긴하게 사용할 수 있을 것이다.

◇Otitis media surgery that outpatients are curious about

 

One of the most frequently asked questions in the outpatient clinic is whether a single otitis media operation will eliminate all ear symptoms. Of course, the symptoms caused by inflammation will improve significantly. However, it should be understood that since the hearing nerve is not a regenerated nerve, it is highly likely that changes have already occurred due to prolonged inflammation, and hearing recovery is often limited if the inflammation is removed by surgery.

 

In some cases, the middle ear space is so inflamed that the inflammation is removed but the hearing improvement is delayed later. In this case, hearing is improved in the second surgery. There is also a question whether it should not be without surgery. Particularly, people who are of high age or who are sick of surgery due to chronic diseases have a strong resistance to surgery.
