식생활 개선이 중요한 역류성 식도염

【한국건강관리협회대구지부 】지삼룡 인제대학교부산백병원 소화기내과 교수=흔히 역류성식도염이라 불리는 위식도역류질환은 서구에서 주로 발생하는 질환이었으나 최근에는 우리나라에서도 방병률이 증가하고 있다.

 

 

위식도역류질환의 흔한 증상은 ‘가슴이 따갑다’또는 ‘쓰리다’라거나 ‘가슴 가운데 부분에 타는 듯한 또는 뜨거운 듯한 불편감과 통증이 있다’라고도 하고 ‘목구멍이나 입으로 산물이나 쓴 물이 올라온다’라고 환자들이 표현한다. 하지 않은 증상이 있을 수도 있는데 ‘가슴이 쓰리다 또는 뻐근하다’, ‘음식을 삼킬 때 걸리거나 잘 내려가지 않는다.’‘목에 뭔가 걸려 있거나 붙어 있는 것 같다’.‘목이 간질간질하거나 목소리를 맑게 하기 위해 헛기침을 한다’ , ‘목이 자주 쉬는 경향이 있다’. ‘명치 아래에 고춧가루를 뿌려놓은 것처럼 화끈거린다’라고 호소하기도 한다.

 

◇ 사회활동을 저해하는 위식도역류질환

 

위식도역류질환의 원인은 가장 중요한 발병 경로인 하부식도 괄약근의 일시적인 이완이며 식도열공 탈장과 같은 해부학적 결손, 하부식도 괄약근의 낮은 압력 등이 원인이 될 수 있다. 하부식도 괄약근은 위와 식도 사이에 있는 조임을 담당하는 근육으로 이 근육이 약화될 경우 위 내 산이 식도로 역류할 수 있다.

 

위식도역류질환의 원인에는 하부식도 괄약근이 중요한 역할을 한다. 위식도역류질환을 흔히 일으키는 원인 중에는 당뇨, 술, 흡연, 비만, 임신 등이 있고 하부식도 괄약근을 약화시키는 요인에는 커피, 탄산음료, 튀김, 기름진 음식, 초콜릿, 케첩, 머스타드, 아스피린 등의 진통소염제 등이 있다.

 

국내 위식도역류질환의 유병률에 대한 연구를 비교해 보면 사십년 사이에 우리나라에서 증가 추세인 것으로 여겨진다. 이는 경제 성장으로 인한 음식·식습관의 변화, 비만 인구의 증가, 고연령층의 증가에서 기안한 것이라 추정된다. 위식도역류질환의 진단은 상부위장관 내시경 검사를 하여 위식도경계부에서 식도염 혹은 식도 점막의 결손을 보는 것이 중요하나 병변이 없더라도 증상만으로 진단이 가능하다.

 

식도산도검사를 할 수도 있다. 위식도역류질환은 삶의 질 저하를 초래하며 신체적인 측면으로 통증과 불편감으로 활동을 피하게 되고 정서적인 측면으로는 병을 갖고 있을지도 모른다는 걱정을 하게 되고 사회 기능적인 측면에서는 어떤 종류의 음식과 외식을 피하거나 다른 사회활동을 기피할 수도 있다. 생산적인 측면으로는 수면장애로 일상 근무에 영향을 미치게 된다.

 

◇ 올바른 식생활습관이 관건

 

위식도역류질환 치료로 양성자펌프억제제를 주로 사용하지만 생활습관을 조절하는 방법이 있다. 일상생활의 조절로서는 비만 환자에서 체중 감량이 도움이 되고, 복부 압력을 높일 수 있는 몸에 조이는 옷은 피하고 몸을 구부리는 동작을 줄인다.

 

 

수면 시 머리 쪽을 올리는 것이 도움이 될 수 있다. 누운 자세에서는 중력이 감소하여 식도로 역류가 쉽게 일어나기 때문이다. 좌측으로 눕는 것이 우측이나 똑바로 눕는 것에 비해 위산 역류를 줄여주는 효과가 있다는 보고도 있다.

 

식사와 관련하여서는 식사 후에 최소 2~3시간 동안은 자리에 눕지 않는 것이 좋다. 가능한 한 저녁은 가볍게 먹는것도 도움이 되며 야식을 하지 않는다. 식후 곧바로 과격한 운동을 하지 않는다. 과식하지 말고 소식을 자주한다. 위산 분비를 촉진하고 식도 점막을 자극하는 고지방 음식, 자극적인 음식, 신 음식, 탄산음료, 커피, 홍차, 카페인, 초콜릿, 술, 담배 등을 피한다.

 

과일 중에는 오렌지나 레몬 같은 감귤류와 파인애플, 포도 등의 신 과일들을 피하는 대신 바나나, 멜론, 배 등과 같이 신맛이 적은 과일류를 선택한다. 고지방 육류보다는 기름이 적은 살코기 부위가 낫고 닭고기도 껍질을 제외하고 먹는 것이 좋다. 등 푸른 생선, 저지방 우유, 요거트, 달걀 등의 고단백 저지방 음식이 추천된다.

 

음식의 종류와 함께 조리법도 중요한데 튀긴 음식은 기름기를 많이 포함하므로 피하고 찌거나 삶거나 그릴 또는 오븐에 구워 먹는 것이 좋다. 또한 스트레스를 줄인다. 스트레스는 위 내에 음식물이 오랫동안 머물도록 하고, 이는 위 내 압력을 증가시켜 역류를 일으킨다.

 

규칙적인 운동은 위 운동에 도움이 되고 위 내 내용물이 잘 배출되도록 하여 위식도역류질환에 도움이 된다. 위식도역류질환에 대해 적절한 식생활습관이란 환자가 좋아하는 특정 기호식품이나 많은 종류의 음식을 반드시 금지하라는 것이 아니다.

 

다만, 환자의 식생활습관 중에서 환자에게 증상을 유발하거나 악화시킬 것으로 의심되는 부분에 대한 개선의 노력이 종종 위식도역류 증상의 불편함을 해소하는데 도움이 될 수 있다. 만약 어떤 음식이 환자의 증상을 유발 또는 악화시키는지 알고 싶다면 1~2주 동안 매일 음식 일기를 적어보는 것도 도움이 된다.

 

◇ 장기적인 관리가 중요한 위식도역류질환

 

위식도역류질환은 약물 요법이 주된 치료이며 위 내 위산감소를 위한 약물로 양성자펌프억제제가 주 치료제이고 히스타민수용체 길항제, 제산제를 사용할 수 있다. 위장운동 촉진을 위한 약물을 사용할 수 있고 기타 보조적인 약물로 항우울제, 항불안제, 점막보호제, 하부식도조임근 작용제를 쓸 수 있다.

 

위식도역류질환은 약을 끊으면 재발을 할 수 있고 장기 유지요법이 필요할 수도 있다. 이 병은 완치보다 관리하는 병이다. 잘 관리하여 증상이 없이 삶의 질이 좋은 상태로 잘 사는 것을 목표로 삼아야 한다.

 

◇외래환자들이 궁금해 하는 중이염 수술

 

외래에서 많이 듣는 질문 중에 하나가 한 번의 중이염 수술로 귀에서 생기는 모든 증상이 없어지냐는 것이다. 물론 염증으로 인한 증상들은 상당 부분 개선될 것이다. 하지만 청력 신경은 재생되는 신경이 아니므로 오랜 염증으로 이미 변화가 왔을 가능성이 높고 수술로 염증을 제거한들 청력 회복은 제한적인 경우가 많다는 것을 이해해야 한다.

 

경우에 따라서는 중이 공간에 염증이 너무 심해서 염증은 제거하되 청력개선을 나중으로 미루기도 한다. 이런 경우는 2차 수술에서 청력을 개선한다. 수술하지 않고 있으면 안되냐는 질문도 있다. 특히 연세가 높거나, 만성질환으로 수술이라면 지긋지긋하신 분들이 수술에 대한 거부감이 심하다. 물론 요즘처럼 달나라에 가는 세상에서 좋은 선택은 아닌 것 같기는 하다.

 

하지만 어차피 선택은 환자의 몫이다. 최소한 합병증이 생길 우려가 있는 환자라면 의사가 극구 수술을 권할 것이지만, 그래도 수술을 선택하지 않을 수는 있다. 하지만 그냥 내버려 두지는 말자. 그렇다고 항생제를 콩알 먹듯 할 수는 없는 터, 좋은 관리 방법을 추천한다. 당장 동네 수퍼마켓에 가서 식용 사과식초를 한 병 사고 약국에서 생리식염수를 한 통 사서 두 액체를 1:1로 섞어서 귓구멍에 적당량 넣어 씻어 내자. 이른바 ‘귀세척’이다.

 

이런 방법으로 염증을 줄이는 건데, 간혹 통증을 호소하는 사람이 있다. 그런 경우라면 1:1이 아니고 식초1에 생리식염수 2를 넣거나 3을 넣어 사용하자. 그리고 혹시 귀세척을 하는 동안에 어지럼이 발생할 수 있으니 희석액은 체온과 같은 36도 정도로 맞춰서 사용하자. 실제로 이 방법은 많은 대형병원에서 귀 수술 이후에 수술 부위 안정을 위해 흔히 쓰는 방법으로 큰 해가 없는 좋은 자가 치료 방법이다. 다만 누군가의 도움으로 귀세척을 한다면, 훨씬 간편한 치료법으로 요긴하게 사용할 수 있을 것이다.
