고통없이 찾아오는 침묵의 병, 췌장암

【한국건강관리협회】김연석 가천대학교 길병원 소화기내과 교수=췌장암은 5년 생존률이 가장 낮은 대표적인 암이다. 하지만 조기진단을 위한 검진이 없어 예방이 쉽지 않다. 췌장암의 증상부터 치료법까지 많은 사람들이 궁금해 하는 췌장암에 대한 주요 정보를 요약해보았다.

 

 

◇복통이 가장 흔한 증상

 

췌장암에서 가장 흔하게 나타나는 증상으로 복통이 있다. 그 외에도 황달, 식욕 감소, 체중 감소 등의 증상이 있고 이보다는 드물지만 지방의 불완전한 소화로 인한지방변이나 혹은 회색변, 구토, 오심 등의 증상이 있을 수 있다. 간혹 당뇨병이 새로 나타나기도 하고 당뇨병을 앓고 있었다면 당뇨병이 악화되기도 한다. 암이 진행하면 등쪽으로 통증을 느끼기도 한다. 위장관 출혈이나 혹은 우울증이나 정서불안 등의 정신 장애, 표재성 혈전성 정맥염, 허약감, 설사, 어지러움 등의 증상이 나타나기도 한다. 황달은 췌장의 머리쪽에 암이 있을 때 대표적으로 나타나는 증상이다. 복통이 있으면서 위-대장 내시경 검사에서 뚜렷한 이상이 없을 경우, 체중이 갑자기 줄면서 황달이 생기는 경우, 허리 통증이 있는데 똑바로 눕기가 힘들지만 옆으로 눕거나 쪼그려 앉으면 통증이 덜한 경우, 당뇨 가족력이 없는데 60세 이후에 당뇨병이 생겼을 경우, 식욕이 없고 식사량도 줄고 식후 복부 통증이 있는 경우, 이런 경우에는 췌장암 검사를 받는 것을 추천한다.

 

◇ 췌장암 진단은 어떻게?

 

췌장암 진단을 위해 복부 초음파를 먼저 받는 경우가 많다. 복부 초음파는 아주 안전한 검사이지만 췌장의 몸통일부나 꼬리쪽은 잘 안 보이는 경우가 있고 배안에 있는 장내 공기 등으로 췌장이 역시 잘 안 보이는 경우가 있다. 복부 초음파 검사에서 이상이 없다고 해도 안심할 수는 없다. 일반적으로 복부 초음파 검사에서 췌장의 종양이 의심되거나 혹은 증상은 췌장암의 가능성이 있지만 복부 초음파 검사에서 확인이 안 된다면 복부 전산화 단층촬영 (복부CT) 검사를 하게 된다. CT검사는 췌장암의 병기 결정(얼마나 퍼졌는가를 확인하는 과정)에 필수적인 검사이다. 췌장의 MRI는 복부 CT검사에 비해 월등한 검사는 아니나 간전이를 확인하는 데는 CT보다 우월하다. 최근에는 췌장암이 의심되나 복부 CT검사에서 종양이 발견되지 않을 경우에 내시경 초음파를 이용하여 췌장암을 진단한다.

 

◇ 어떤 치료법이 있나?

 

췌장암은 주요 혈관의 침범이나 전이 여부에 따라 절제 가능 췌장암, 경계성 절제 가능 췌장암, 국소 진행 췌장암, 전이 췌장암으로 구분을 하게 된다. 영상 소견에서 췌장암 가능성이 매우 높고 절제가 가능하다고 판단된다면 수술 전에 조직 검사를 하지 않고 절제술을 시행한다. 그러나 환자의 전신 상태가 좋지 않아 수술을 받지 못하거나 혹은 암 진단 시 중요 정맥 또는 동맥 침범이 있는 경우 혹은 간이나 복강 혹은 기타 장기에 전이가 있는 경우에는 항암 화학 요법이나 방사선 요법을 계획해서 치료를 하게 된다. 이와 같은 경우에는 조직 검사를 시행하여 췌장암을 확진하는 것이 필요하다. 경계성 절제 가능 췌장암의 경우에는 신보조적 항암 요법(neoadjuvant chemotherapy)을 시행하며 치료에 잘 반응하여 암의 크기가 줄어들면 절제 수술을 시도하는 경우가 있다.

 

◇최근 몇 년 사이 완치율 높아져

 

췌장암은 알려진 대로 5년 생존율이 가장 좋지 않은 대표적인 암이다. 근치적 절제가 가능한 경우에도 평균 생존 기간은 13~20개월 정도이고 이 경우도 20%의 환자에서만 장기 생존을 보였다. 국소적으로 진행된 경우에는 평균 생존 기간이 6~10개월 정도다. 간 또는 기타 원격 장기에 전이가 있는 경우에는 평균 생존 기간이 약 6개월에 불과하다. 그러나 최근 몇 년 사이에 췌장암의 치료성적이 많이 좋아지고 있다. 1~2기 췌장암의 경우에는 국내의 중앙암등록본부 발표를 보면 2012~2016년 기간 동안 국내 1~2기 췌장암 완치율이 29.7% 즉 3명 중 1명은 생명을 건질 수 있다. 1기의 경우에는 완치율이 50%정도까지 올라가며 1기 중에서도 발견이 어렵지만 1cm미만의 췌장암의 경우에는 완치율이 70%정도 수준으로 올라간다.

 

◇췌장암의 진단 및 예방을 위한 방법

 

위암이나 자궁암처럼 일반적으로 조기 진단을 위한 검진은 없다. 그러나 70세 이상 노인, 10년 이상 장기 흡연자, 만성 췌장염 환자, 50세 이후 가족력 없이 갑자기 당뇨가 생긴 경우, 췌장암 가족력이 있는 경우에는 췌장암의 고위험군이므로 정기적인 추적 검사가 필요하다. 최근 췌장 물혹이 있는 경우가 많이 발생하는데, 췌장 물혹의 경우 정상인에 비해 췌장암이 발생하는 위험성이 약 10배정도 더 높은 것으로 알려져 있다. 일반적으로는 복부CT나 복부 MRI 혹은 내시경 초음파 검사를 혈청 종양 표지자 검사와 함께 일년에 한 번 정도 받는 것을 추천하고 있다. 췌장암을 예방하기 위한 뚜렷한 예방 수칙이나 권고 기준은 없다. 단, 금연이나 식습관 개선 등을 통해 위험 요인으로 알려진 것들을 일상생활에서 회피하여 예방할 것을 권장한다.

◇ How is pancreatic cancer diagnosed?

 

In many cases, abdominal ultrasound is received first to diagnose pancreatic cancer. Abdominal ultrasound is a very safe test, but there are cases where the body or tail of the pancreas is difficult to see, and the pancreas is also difficult to see due to intestinal air in the stomach. Even if there are no abnormalities on the ultrasound of the abdomen, it is not safe to say. If the possibility of pancreatic cancer is very high from the imaging findings and it is determined that resection is possible, resection is performed without biopsy before surgery. However, if the patient's systemic condition is poor and cannot receive surgery, or if there is significant venous or arterial invasion during the diagnosis of cancer, or if there is metastasis to the liver, abdominal cavity or other organs, chemotherapy or radiation therapy should be planned and treated. do. In such a case, it is necessary to perform a biopsy to confirm pancreatic cancer. In the case of borderline resectable pancreatic cancer, neoadjuvant chemotherapy is performed, and resection surgery is sometimes attempted if the size of the cancer is reduced by responding well to treatment.

 

◇The cure rate has increased in recent years

 

Pancreatic cancer, as known, is a representative cancer with the worst 5-year survival rate. Even when radical resection was possible, the average survival period was about 13-20 months, and in this case, only 20% of patients showed long-term survival. In the case of local progression, the average survival period is about 6-10 months. For metastases in the liver or other distant organs, the average survival time is only about 6 months. However, in recent years, the treatment results of pancreatic cancer have improved a lot. ◇ Methods for diagnosis and prevention of pancreatic cancer

 

There is generally no screening for early diagnosis like stomach or uterine cancer. However, if the elderly over 70 years old, long-term smokers over 10 years, chronic pancreatitis patients, sudden diabetes without a family history after 50 years of age, or have a family history of pancreatic cancer, they are at high risk for pancreatic cancer, so regular follow-up is necessary. Recently, there are many cases of pancreatic lumps, and it is known that pancreatic lumps have a risk of developing pancreatic cancer about 10 times higher than that of normal people. In general, it is recommended to undergo abdominal CT, abdominal MRI, or endoscopic ultrasonography along with serum tumor markers about once a year. There are no clear precautions or recommended criteria for preventing pancreatic cancer. However, it is recommended to avoid and prevent known risk factors in daily life through smoking cessation or improving eating habits.
