영남대병원 전익찬 교수팀, 척추감염 치료판정 연구성과 발표

【브레이크뉴스 대구】이성현 기자=영남대병원(병원장 김성호)은 신경외과 전익찬 교수팀이 최근 일체형 암진단기 PET-MR을 이용한 새로운 패러다임의 척추감염 치료판정 이론을 국제 저명학술지에 발표했다고 16일 밝혔다.

해당 연구는 2019년부터 영남대병원에서 수행 중인 과학기술정보통신부, 보건복지부 주관 혁신형 의사과학자 공동연구사업(단장 원규장)의 세부과제로 선정되어 신경외과 전익찬 교수, 핵의학과 공은정 교수 그리고 대구가톨릭대 방사선학과 홍철표 교수의 협업으로 진행되고 있다.

 

전익찬 교수팀은 그간 학설로 존재하던 ‘척추감염 치료판정에 PET-MR의 활용 가능성’과 관련하여 실제 환자에 적용한 사례들을 2019년부터 국제 저명학술지에 여러 차례 보고하였으며, 최근 SCI급 국제 저명학술지 중 하나인 「Diagnostics」(IF 3.11, Q1)에 대규모 화농성 척추감염 환자들을 대상으로 한 임상 연구 논문 2편을 연이어 게재하며, 그 학술적 성과를 인정받았다.

 

척추감염은 척추에 세균성 또는 결핵성 감염이 발생하는 질환으로 최근 고령 환자에서 퇴행성 척추질환의 치료 합병증이나 다양한 기저 질환과 연관하여 그 발생 빈도가 급격히 증가했다. 척추감염이 발생하는 경우 극심한 통증이 발생하며 일상생활이 불가능해지고 이에 대한 치료로 장기간 항생제 주사 또는 항결핵제의 복용이 필요하다. 하지만 아직 척추감염에 대한 정확한 치료 가이드라인이 확립되어 있지 않고 국가별로 원인균과 약제 내성률이 달라서 치료에 많은 어려움이 있다. 장기간 항생제 또는 항결핵제 치료 시 약제와 관련한 합병증으로 2차적 문제점이 발생할 가능성이 높아 치료 중 잔존병변의 여부에 대한 정확한 판정은 불필요한 약물의 사용과 관련하여 합병증을 줄이는 데에 중요하다.

 

현재 혈액검사로 측정하는 염증수치는 다른 신체컨디션의 영향을 많이 받으며, 척추감염병변의 상태에 따라 그 변화가 심하여 정확도가 낮은 단점이 있다. 약 10여 년 전부터 척추감염 환자의 치료판정에 PET-CT를 이용한 일부 연구가 있었으나 실제 환자 치료 시 적용이 어려우며, PET에 CT를 결합하여 높은 방사선 피폭과 척추병변에 대한 해부학적 해상도가 낮은 단점이 있었다.

 

하지만 영남대병원에서 운용 중인 일체형 PET-MR은 체내 세포의 생화학적인 변화를 영상으로 표현하는 PET의 장점과 방사선 피폭 없이 척추의 해부학적 상태를 정확하게 나타내는 MRI를 동시에 적용하여 척추감염 병변의 감염 상태와 척추의 구조적 변화를 확인하는데 현존하는 최상의 기기로 인정받고 있다.

 

전익찬 교수는 연구에서 이러한 PET-MR의 장점을 척추감염 치료판정에 적용하여 기존 혈액검사 등의 방법들 보다 잔존병변의 진단에 더 높은 정확성을 확인했다. 이는 불필요한 항생제 사용과 관련 합병증, 전체적인 재발률을 줄이는 성과로 이어졌다. 그리고 척추감염 치료판정에 PET-MR의 적용 방법과 결과의 해석에 대한 기본적인 이론을 정립하여 추가연구가 수행될 수 있는 토대를 마련하였다. 또한, 이번 연구 성과는 암질환의 검진과 치료판정에 주로 사용되던 PET-MR의 활용도를 다른 영역으로 확대했다는 점과 PET을 기반으로 MRI의 정확한 변화를 확인하여 다양한 골감염 질환의 치료판정에서 MRI 단일장비의 한계를 극복할 가능성을 열었다는 점에서 또 다른 의미가 있다.

 

전익찬 교수는 “본 연구에 참여한 대부분의 환자들은 척추감염 진단을 받고 치료에 어려움을 겪던 중 본원으로 오셔서 흔쾌히 연구 참여를 수락하셨다. 미래에 같은 질환으로 고통을 받을 다른 환자를 위해 많은 기여를 하셨고, 감사하게 생각한다. 현재 인공지능을 적용하여 개발 중인 ‘PET 연동 특수 MRI 영상(알파시퀀스) 변환 프로그램’을 조기에 완성하여 향후 PET이 없는 의료기관에서도 이를 사용할 수 있도록 하겠다”는 계획을 밝혔다.

 

전익찬 교수는 현재 척추감염 외에도 퇴행성과 외상성 척추 질환에 대한 수술 및 비수술적 치료를 시행하고 있으며, 특히 척추신경종양, 전이성척추암, 재수술이 필요한 척추변형 환자 치료에 중점을 두고 있다.

 

한편, 영남대병원은 2012년 국내 최초로 암 진단과 치료판정을 위해 최첨단 일체형 PET-MR를 도입했다. PET-CT와 비교하여 방사선 피폭량은 대폭 줄이고 검사의 질을 향상했다. 낮은 방사선 피폭 및 뇌와 척추 질환과 같이 MRI가 꼭 필요한 영역에서 장점이 높아 호평을 받고 있다.
