【한국건강관리협회】박정환 한양대학교병원 내분비대사내과 교수=우리는 비만을 미용상의 문제나 다른 질병을 일으키는 위험요소 정도로 단순하게 인식해 왔다. 그러나 비만 환자와 비만 관련 질병이 급격히 늘어나고 이와 관련된 의료비용의 지출도 꾸준히 증가하면서 최근에는 비만을 독립된 질병으로 생각하고 있다.

 

 

우리나라도 비만 환자가 꾸준히 증가하고 있다. 이러한 이유 중 하나는 우리가 칼로리 과잉 시대에 살고 있기 때문이다. 지금과 같이 풍족한 생활을 하기 시작한 것은 약 30년 전부터다. 이전까지는 계속해서 영양이 부족한 환경에서 살아야 했기 때문에 우리의 한정된 몸 안에 더 많은 에너지를 저장하는 것이 필요했다.

 

따라서 우리 몸은 에너지 효율이 높은 지방으로 에너지를 저장하는 방식을, 생존을 위해 지방을 아끼는 방향으로 진화했다. 이러한 몸은 단시간 내에 변화하지 않기 때문에 칼로리 과잉 시대에 살아가는 요즈음의 우리는 비만이라는 새로운 질병에 쉽게 노출되고 있는 것이다.

 

◇마른비만 가늠하는 허리둘레

 

비만이라고 하면 보통 몸무게가 많이 나가는 것이라고 단순하게 생각하는 사람들이 있다. 하지만 정확히는 지방이 정상보다 더 많이 축적된 상태이다. 따라서 몸무게가 많이 나가지 않더라도 비만일 수 있다. 비만은 지방이 정상보다 많이 축적된 상태이므로 체내 지방량을 측정하는 것이 비만을 진단하는 가장 정확한 방법이다.

 

그러나 실제 지방량을 정확히 측정하기가 어렵기 때문에 일반적으로 체질량지수와 허리둘레 측정을 통해서 진단하게 된다. 체질량지수는 자신의 몸무게(kg)를 키의 제곱(㎡)으로 나눈 값이다. 우리나라의 경우 체질량지수가 25kg/㎡ 이상인 경우에 비만이라고 진단한다.

 

그러나 체질량지수는 운동선수 등 근육량이 많은 경우 체지방이 많지 않아도 비만으로 진단될 수 있는 문제점이 있다. 체질량지수의 이러한 단점을 보완할 수 있는 방법이 허리둘레 측정이다. 우리 몸에 축적된 지방에는 피하지방과 내장지방이 있다. 내장지방이 피하지방보다 비만 관련 질병에 더 큰 영향을 미치는 것으로 알려져 있다.

 

우리가 흔히 ‘마른 비만’이라고 얘기하는 ‘말라도 배만 볼록 나온 사람’은 내장지방이 많은 상태일 수 있다. 이러한 내장지방의 과다 여부를 허리둘레를 통해 알 수 있다. 허리둘레는 숨을 편안히 내쉰 상태에서 줄자를 이용하여 측정한다.

 

측정 위치는 갈비뼈 가장 아래 위치와 골반의 가장 높은 위치의 중간부위를 측정한다. 측정 시에는 줄자가 연부조직에 압력을 주지 않을 정도로 느슨하게 하여 0.1cm까지 측정한다. 우리나라의 경우 성인 남자에서는 90cm 이상, 여자에서는 85cm 이상일 때 내장지방이 많은 복부비만으로 진단한다.

 

◇ ‘저탄고지 식단’이 비만을 유발한다?

 

비만은 일차성 비만과 이차성 비만으로 나눈다. 일차성 비만은 에너지 섭취량이 에너지 소모량보다 많은 상태에서 체지방이 증가하여 발생하며, 이차성 비만은 유전, 내분비질환, 약제 등에 의해서 발생한다. 비만의 90% 이상은 칼로리 과잉과 연관된 일차성 비만이다.

 

일차성 비만은 유전적 요인과 환경적 요인이 상호작용하여 발생한다. 부모 모두가 비만할 경우 80%, 부모 한 명이 비만할 경우는 40%, 부모 모두가 비만하지 않을 경우는 7%에서 자녀가 비만할 수 있다. 그러나 비만 환자의 2/3는 어렸을 때는 비만하지 않았는데 성인이 되어서 비만해진 것으로 알려져 환경적 요인이 많이 관여하는 것으로 생각된다.

 

환경적 요인 중 첫 번째 원인은 과식을 포함한 잘못된 식사종류와 습관이다. 다이어트에 관심이 높아지면서 ‘저탄고지 식사’ 등을 시도하는 사람들이 늘어나고 있다. 저탄고지 식사의 경우 초기에 탄수화물 섭취가 줄면서 체중감소 효과가 있지만 궁극적으로는 지방 함유량이 많은 음식의 잦은 섭취는 비만의 원인이 된다.

 

최근에 ‘흑당’ 열풍이 불었는데, 흑당 역시 조금의 차이는 있지만 설탕과 같은 단순당으로, 과도한 섭취는 문제가 될 수 있다. 단순당을 많이 포함한 음료, 과자, 음식 등을 섭취하게 되면 곡물 등의 다당류의 탄수화물보다 빠르게 몸에 흡수되면서 지방 축적의 원인이 된다. 특히 과도한 당분 섭취는 점차 중독성을 나타내 당분을 더 많이 섭취하려는 경향을 보이게 되어 최근에 소아 청소년 비만의 중요한 원인으로 지적되고 있다.

 

◇ 사망위험 높이는 비만

 

비만은 비만으로 그치는 것이 아니라 각종 질병의 원인이 될 수 있다. 2형 당뇨병, 이상지질혈증, 고혈압, 지방간, 담낭질환, 협심증이나 심근경색증과 같은 관상동맥질환, 뇌졸중, 수면무호흡증, 통풍, 골관절염, 월경이상, 대장암, 유방암 등이 대표적인 비만과 관련된 질병들이다.

 

또한 비만은 사망의 위험을 20% 증가시키는 것으로 알려져 있으며, 체질량지수 및 허리둘레가 증가할수록 사망률의 위험이 높아진다. 식사조절, 운동 및 행동조절의 병합은 비만의 치료 및 예방에 있어서 가장 효과적인 방법이다. 약물치료나 수술치료가 필요한 경우에도 생활습관의 관리는 반드시 필요하다.

 

식사치료 시에는 에너지 섭취량을 줄이면서 필수 영양소는 충분히 섭취하며, 목표 체중으로의 감량을 목표로 한다. 운동의 체중감량 효과는 다른 방법에 비해 상대적으로 적지만 비만 관련 질환의 유병률을 줄이고 건강과 관련된 많은 추가적인 이익을 제공한다.

 

운동은 주 3회 고강도 운동을 주당 200분 이상, 또는 2,500kcal 이상을 소비하는 유산소와 저항운동을 실시해야 한다. 약물치료는 비만의 식사치료나 운동 등의 비약물치료를 대신할 수 없으며, 생활습관 교정을 시행하면서 보조적으로 시행해야 한다.

◇ Waist circumference that measures only thin obesity

 

Some people simply think that when it comes to obesity, you usually weigh a lot. But to be precise, more fat has accumulated than normal. So even if you don't weigh a lot, you can be obese. Obesity is a state in which fat is accumulated more than normal, so measuring the amount of fat in the body is the most accurate way to diagnose obesity.


However, since it is difficult to accurately measure the actual fat mass, it is generally diagnosed through body mass index and waist measurement. Body mass index is the value obtained by dividing one's body weight (kg) by the square of height (㎡). In Korea, if the body mass index is 25kg/㎡ or more, it is diagnosed as obesity.


However, there is a problem in that the body mass index can be diagnosed with obesity even if there is not much body fat in case of a large amount of muscle such as an athlete. A way to compensate for this shortcoming of body mass index is waist measurement. Fat accumulated in our body includes subcutaneous fat and visceral fat. Visceral fat is known to have a greater effect on obesity-related diseases than subcutaneous fat.


“People who have only a skinny stomach,” often referred to as “dry obesity,” may have a lot of visceral fat. Whether or not there is an excess of visceral fat can be seen through the waist circumference. The waist circumference is measured with a tape measure while exhaling comfortably.


As for the measurement position, measure the middle of the lowest position of the ribs and the highest position of the pelvis. When measuring, loosen the tape measure to the extent that it does not apply pressure to the soft tissue and measure it to 0.1cm. In Korea, when it is 90cm or more in adult males and 85cm in females, it is diagnosed as abdominal obesity with a lot of visceral fat.

 

◇ “Low-tango diet” causes obesity?

 

Obesity is divided into primary obesity and secondary obesity. Primary obesity is caused by an increase in body fat when energy intake is greater than energy consumption, and secondary obesity is caused by inheritance, endocrine diseases, and drugs. Over 90% of obesity is primary obesity associated with calorie excess.


Primary obesity is caused by the interaction of genetic and environmental factors. Children can be obese in 80% if all parents are obese, 40% if one parent is obese, and 7% if all parents are not obese. However, two-thirds of obese patients were not obese when they were young, but it is known that they became obese when they became adults, and environmental factors are thought to be involved a lot.

 

The first of the environmental factors is the wrong diet type and habit, including overeating. With increasing interest in diet, more and more people are trying to “low-tango diet”. In the case of low-tango diets, carbohydrate intake is reduced in the early stage, which is effective in weight loss, but ultimately, frequent intake of foods high in fat is the cause of obesity.


Recently there has been a craze for “brown sugar,” but brown sugar is also a simple sugar like sugar, and excessive consumption can be a problem. Ingestion of beverages, snacks, and foods containing a lot of simple sugars is absorbed into the body faster than carbohydrates of polysaccharides such as grains, causing fat accumulation. In particular, excessive sugar intake is gradually addictive and tends to consume more sugar, which has recently been pointed out as an important cause of childhood obesity.

 

◇ Obesity increases the risk of death

 

Obesity does not stop with obesity, it can cause various diseases. Type 2 diabetes, dyslipidemia, high blood pressure, fatty liver, gallbladder disease, coronary artery disease such as angina pectoris or myocardial infarction, stroke, sleep apnea, gout, osteoarthritis, menstrual abnormalities, colon cancer, and breast cancer are typical obesity-related diseases.


In addition, obesity is known to increase the risk of death by 20%, and the risk of mortality increases as body mass index and waist circumference increase. The combination of diet control, exercise and behavior control is the most effective method in the treatment and prevention of obesity. Even if medication or surgical treatment is needed, management of lifestyle is essential.


During meal treatment, reduce energy intake while ingesting enough essential nutrients, and aim to lose weight to target. Although the weight loss effect of exercise is relatively small compared to other methods, it reduces the prevalence of obesity-related diseases and provides many additional health-related benefits.


Exercise 3 times a week high-intensity exercise, 200 minutes or more per week, or 2,500 kcal or more consuming aerobic and resistance exercise should be performed. Drug treatment cannot replace non-drug treatment such as diet or exercise for obesity, and should be performed as an aid while performing lifestyle correction.
