【브레이크뉴스 포항】박영재 기자=국내 연구진이 사람과 비슷한 토끼의 결막 술잔세포를 영상으로 검사하는 데 성공했다. 특히 안구건조증 토끼 모델에서 손상부터 회복까지를 추적한 이 연구성과는 향후 사람의 안구건조증 진단‧치료에 유용하게 활용될 수 있다.
이 연구성과는 안과 분야의 권위 있는 국제 학술지 ‘The Ocular Surface’에 최근 게재됐다.
안구 표면에 점액을 분비하는 결막 술잔세포는 눈물막의 뮤신층을 형성하고 눈물막을 유지하는 역할을 한다. 이 술잔세포의 기능에 이상이 생기면 안구건조증을 포함한 다양한 안구 표면질환이 생기거나 악화될 수 있다. 이 때문에 술잔세포 검사는 안구 표면질환을 정확하게 진단하고 치료하는 데 필수적이지만, 아직까지 적절한 검사법이 없었다.
앞서 연구팀은 안과 항생제인 목시플록사신이 결막 술잔세포를 염색한다는 것을 세계 최초로 발견하고, 고심도 형광현미경을 개발해 살아있는 쥐의 눈에서 결막 술잔세포를 영상화하는 데 성공한 바 있다.
이번 연구에서는 사람의 눈에 적용하기 전 단계로 사람의 눈과 비슷한 토끼 눈에서 결막 술잔세포 영상화를 통한 비침습 검사법을 시도했다.
먼저, 정상 토끼에서 결막 표면에 골고루 분포하고 있는 술잔세포의 실시간 영상을 확인할 수 있었다. 이 영상은 조직을 채취해 병리 조직 프로세스를 통해 얻는 술잔세포 영상과 완전히 일치했다.
또, 연구진은 해당 검사법이 안구건조증이 있는 토끼에서도 결막 술잔세포 변화를 탐지할 수 있는지 연구했다. 안구건조증은 안과 수술에 사용하는 소독제인 포비돈 요오드(povidone-iodine, 제품명 베타딘(Betadine))을 점안함으로써 유도했는데, 이는 안과 수술 후 일시적으로 건조증이 발생함에 착안한 것이다.
소독제로 손상된 결막에서 술잔세포 밀도의 감소를 관찰했고 같은 토끼를 3주간 더 관찰해 술잔세포 밀도가 다시 증가하는 사실을 확인했다. 이 검사법으로 관찰한 토끼모델의 술잔세포 밀도 변화는 함께 시행한 안구건조증 측정법(눈물막 파괴시간, 눈물 양 측정(쉬르머 검사))의 결과들과 일치했다.
이 기술은 안구 표면질환 환자의 치료와 예방에 사용하는 목시플록사신 항생제를 사용하므로 환자에게도 부담 없이 적용할 수 있다.
POSTECH 김기현 교수는 “비침습 결막 술잔세포 영상법이 사람과 비슷한 토끼 모델에서도 잘 동작함을 검증했다”며 “향후 임상시험을 통해 검증해 안구 표면질환 환자의 정밀 진단과 치료에 활용될 수 있도록 하겠다”고 말했다.
이 연구는 정보통신기획평가원 (IITP) ICT R&D 혁신 바우처지원사업의 지원을 받아 진행됐다.

Conjunctival goblet cells that secrete mucus on the surface of the eyeball form the mucin layer of the tear film and play a role in maintaining the tear film. If the function of these goblet cells is abnormal, various ocular surface diseases, including dry eye syndrome, may occur or worsen. For this reason, goblet cell testing is essential for the accurate diagnosis and treatment of ocular surface diseases, but there is no suitable test method yet.
Previously, the research team discovered for the first time in the world that the ophthalmic antibiotic moxifloxacin stains conjunctival goblet cells, and developed a high-depth fluorescence microscope to image conjunctival goblet cells in the eyes of live mice.
In this study, a non-invasive test using conjunctival goblet cell imaging was attempted in rabbit eyes similar to human eyes as a step before application to human eyes. First, it was possible to confirm the real-time image of goblet cells evenly distributed on the surface of the conjunctiva in normal rabbits. This image was completely consistent with the goblet cell image obtained from tissue extraction and pathological tissue processing.
The researchers also studied whether the test could detect changes in conjunctival goblet cells in rabbits with dry eye syndrome. Dry eye syndrome was induced by instillation of povidone-iodine (product name: Betadine), a disinfectant used for ophthalmic surgery, based on the temporary dryness occurring after ophthalmic surgery.
A decrease in goblet cell density was observed in the conjunctiva damaged by the disinfectant, and the same rabbit was observed for 3 more weeks, confirming that the goblet cell density increased again. The change in goblet cell density in the rabbit model observed with this test method was consistent with the results of the dry eye syndrome measurement method (tear film breakage time, tear volume measurement (Schimmer's test)).
Since this technology uses moxifloxacin antibiotics used for treatment and prevention of ocular surface disease patients, it can be applied to patients without burden.
Professor Kihyun Kim of POSTECH said, “We have verified that the non-invasive conjunctival goblet cell imaging method works well in a rabbit model similar to humans. ” he said.
