위암의 예방과 관리

【브레이크뉴스 】박은정 기자= 우리나라는 위암 발병률이 높아 40세 이후부터 2년마다 위장조영검사나 위내시경 검사를 받을 것을 권유하고 있다. 

 

또 위내시경 검사에서 위암 위험도가 높다는 소견이 나오면 정기적으로 위내시경 검사를 받아야 한다. 이 검사에서는 주로 만성 위축성 위염, 장상피화생, 위궤양, 위의 선종성 용종 등을 확인할 수 있다. 위암의 예방과 관리에 대해 알아본다.

 

위암의 발병 원인은 명확하게 밝혀지지 않았지만, 식습관, 환경이나 유전, 문화적 요인들이 있다. 위암 유발인자로는 헬리코박터균, 흡연, 술, 가족력, 짠 음식 등이 꼽힌다. 

 

세계보건기구에서는 위산 속에서도 살 수 있는 나선형 세균인 헬리코박터균을 위암을 일으키는 발암물질로 분류했다.

 

헬리코박터균 감염이 위암 발생에 독립적으로 관여한다고 인정하기에는 아직 의학적 증거가 불충분하지만, 여러 대규모 역학연구에서 헬리코박터균 감염률이 높은 나라에서 위암 발병률이 상대적으로 높다고 한다. 헬리코박터균은 위염, 위암 등을 일으키는 주요 원인으로, 위암 발생 위험을 3.8배 증가시킨다.

 

위암은 흡연과도 관련이 있다. 흡연자는 비흡연자보다 위암 발병 위험도가 3배 정도 높다. 

 

우리나라에서 남녀 간 식생활 차이가 별로 없음에도 남자의 위암 발생률이 여자보다 2배 가량 높은 것은 남성의 흡연율이 여성보다 높다는 사실과 연관된 것으로 보인다. 물론 이 외에도 음주 등 다른 환경적 요인이 있지만, 담배는 가장 잘 알려진 발암 원인이다. 흡연은 삼가는 게 좋다.

 

술은 간에 영향을 많이 준다. 과음을 하면 위암 발생 위험이 2배가량 높아진다. 위 점막을 손상시키고 만성위염을 유발해 최종적으로 위암이 발생할 수 있다고 하니 음주를 하더라도 시간을 두고 소량을 천천히 마시도록 한다.

 

모든 질환이나 암이 그렇듯이 가족 중에 위암 환자가 있는 경우 위암 발생률이 2배로 증가한다. 

 

유전적 요인도 있지만, 가족의 생활환경과 식습관이 비슷해서 생기는 것이라고 해석된다. 짠 음식을 많이 섭취한 사람은 적게 섭취한 사람보다 위암 발병 위험도가 4.5배가량 높다.

 

질산염 화합물(가공된 햄, 소시지류 등 가공보관 식품), 탄 음식, 염장 식품들도 위암 발생 위험을 높인다고 한다. 지나친 염분은 위점막을 자극하기 쉽고, 자극과 손상이 지속되면서 위암이 발생하기 쉬운 환경을 만든다. 매운 음식도 위산을 다량 분비하게 해서 위에 자극을 주고 위염과 위궤양 등을 유발하게 된다.

 

위암 치료법

 

수술로 원발 병소를 완전히 절제하고, 위 주위의 광범위한 림프절절제술을 시행한 후 위장관 재건술을 한다. 수술은 병변 위치와 침윤 정도에 따라 위아전절제술(2/3 절제), 위전절제술 및 합병 절제와 함께 위 주위의 광범위한 림프절절제술을 함께 시행한다.

 

일부 국한성 표재성 위암에는 내시경 점막절제술이나 내시경 점막하박리술을 시도한다. 여러 메타분석에서 보조 항암화학요법이 생존율을 높이는 것으로 나타나고 있어, 위암의 근치적 수술 후 보조 항암화학요법을 권고한다.

 

수술 전 방사선치료는 국소적으로 시행하는 위암의 근치 절제 가능성을 높이기 위해 제한적으로 수술 전에 시행한다. 또 위암의 근치 수술 후 보조요법으로 항암화학, 방사선 병용요법을 고려할 수 있다.

 

정기 건강검진과 올바른 식습관으로 예방

 

40세부터 2년에 한 번 권고되는 위내시경 검사는 위암 발병 여부를 확인하는 위암 예방법이라고 할 수 있다. 

 

위암 환자 대부분이 초기에는 특별한 증상이 없었고 우연한 기회에 암을 발견했다고 하는 만큼 위내시경 검사를 정기적으로 받는 것이 매우 중요하다. 위암은 초기에 발견하면 치료가 매우 수월해지기 때문이다. 위축성위염과 장상피화생이 있다면 매년 위내시경 검사를 받는 것이 좋다.

 

 위암을 예방하는 올바른 식습관

 

 -훈제식품을 적게 먹고, 태운 육류나 생선 등을 먹지 말 것

 -소금에 절인 식품, 짠 음식은 피할 것

 -방부제나 식용색소가 적게 든 음식물을 선택할 것

 -딱딱하거나 뜨거운 음식 섭취를 삼갈 것

 -녹황색 채소와 과일을 많이 먹을 것

Although there is not much difference in diet between men and women in Korea, the fact that the incidence of gastric cancer in men is twice higher than that of women seems to be related to the fact that the smoking rate of men is higher than that of women. Gastroscopy, which is recommended every two years from the age of 40, is a method of preventing gastric cancer to check for gastric cancer.

 

It is very important to receive regular gastroscopy because most gastric cancer patients do not have any specific symptoms in the early stages and are diagnosed with cancer by chance.
