【브레이크뉴스 대구】박은정 기자=아이가 흉통을 호소하면 큰 병은 아닌지 걱정이 앞선다. 흉통 하면 심장 이상을 떠올리게 되고 응급 상황으로 생각하기 쉽기 때문이다. 

 

그러나 소아·청소년기에 흉부 불편감의 가장 흔한 원인은 근골계나 소화기, 심리적 문제가 가장 많고 심장이 원인인 경우는 5% 미만이라고 하니 일단 안심이다. 흉통의 원인 질환에 대해 알아본다.

 

최근 코로나19 백신 접종 후 가슴통증을 호소하며 응급실에 내원하는 아이들이 늘었다. 

 

화이자사의 코로나19 백신인 코미나티주® 접종 후 소아·청소년에서 부작용으로 심근염 위험도가 일부 높다는 통계도 있으나, 코로나19 이전 통계에서는 응급실을 찾은 전체 소아·청소년 환자 중 약 0.5% 정도가 흉부 불편감을 원인으로 내원하여 많은 수는 아니었다. 

 

다만 통증은 주관적인 증상이라 실제로 어떤 질환인지, 얼마나 위험한지 정확히 알기 어려운 경우가 많다.

 

또 흉통 하면 일반적으로 심장 문제를 떠올리기 때문에 아이가 가슴이 불편하다고 하면 보호자는 심각한 건강문제로 여겨 응급실을 찾게 된다. 

 

결론부터 말하자면 흉통으로 응급실을 방문한 환아 중 심장 이상은 1% 미만이며, 소아·청소년에서 흉부 불편감의 대부분은 심장 문제가 아니다. 하지만 일부 심폐질환의 경우 위험할 수도 있으니 어떤 때에 응급실을 방문해야 하는지 알아보자.

 

흉통이 있을 때 응급실로 가야 하는지의 판단 기준은 갑작스럽고 심한 가슴통증이 오래 지속될 때이다. 특히 운동이나 활동 시 평소 없던 통증이 나타난다면 응급실을 방문하는 것이 좋다.

 

 또 호흡곤란이나 빠른 호흡, 발열, 실신, 두근거림 등이 동반된다면 빨리 원인을 찾아야 한다. 아이들은 성인과 달리 선천성심장병 등 기저질환이 없다면 심근경색이나 심부전 등의 발생 비율은 매우 낮다.

 

주의해야 할 원인은 감염이나 면역반응으로 인한 심근염이나 심장막염 등이 있으며, 생명이 위독할 정도로 위험할 수도 있다. 

 

다행인 것은 소아·청소년기에 흉부 불편감의 가장 흔한 원인은 근골격계나 소화기계통, 심리적 문제가 가장 많고 심장이 원인인 경우는 5% 미만으로 많지 않다는 점이다.

 

심장 이상으로 발생하는 흉통의 원인

 

각각의 증상에 따른 원인 질환을 살펴보면 통증의 위치와 빈도, 기간 및 강도, 동반증상 그리고 어떤 상황에서 나타나는지가 중요하다. 먼저 흉통의 원인인 심근염은 심장근육에 염증이나 감염으로 인해 심장 기능이 떨어지는 질환을 말한다. 

 

소아에서는 주로 1~2주 전 감기를 앓고 난 후 발병하는데 면역기능 이상, 항암제 등 약물에 의해서도 나타날 수 있다.

 

심근육이 약해져 심박출량이 떨어지거나 부정맥이 발생할 수 있어 흉통을 호소하며 식은땀을 흘리기도 하고 심한 경우 심장비대에 폐성심으로 숨이 가쁘고 가슴이 함몰될 정도로 헐떡이며 실신할 수도 있다. 

 

감염이 원인일 경우 발열이 동반될 수도 있으나 열 없이도 나타날 수 있다. 심한 경우 체외막 산소공급치료나 심장이식까지 필요할 수도 있으므로 이러한 증상이 있으면 바로 응급실로 가야 한다.

 

심장막염은 심장을 둘러싼 얇은 막에 염증이 생기는 것으로, 여러 가지 원인에 의해 발생하지만, 소아의 경우 심근염과 마찬가지로 감염이나 염증에 의한 것이 가장 많다. 

 

수 시간에서 수일간 좌측 가슴에 심한 통증이 생겨 팔이나 목, 등으로 퍼지면서 크게 숨을 쉬거나 기침 시 심해지고, 앉거나 앞으로 허리를 구부리면 통증이 완화되고 누웠을 때 악화되면 의심할 수 있다. 염증이 심해 심장막액의 양이 증가하면 심막 삼출이 생겨 심낭압전(심장눌림증)으로 실신이나 쇼크를 유발할 수 있어 위험하다.

 

그 밖에 심장 원인으로는 가와사키병(Kawasaki’s disease)을 앓았던 아이가 운동 시 실신을 하거나 갑자기 심한 좌측 흉통을 호소한다면 관상동맥합병증으로 인한 협심증이나 심근경색을 생각할 수 있다. 

 

또 키가 크고 마르며 상지가 하지보다 긴 마르판증후군(Marfan syndrome) 환아에서 갑자기 찢어지는 듯한 극심한 흉통이 가슴과 등쪽에 나타난다면 대동맥 박리 소견으로 응급 수술이 필요할 수도 있다.

 

갑자기 가슴이 답답하고 심하게 두근거린다면 소아 부정맥일 수 있으며, 발작성 상심실성 빈맥이 가장 흔하다. 보통 50~200회 정도의 심박동수를 보이며 환아가 시작과 끝남을 알 수 있을 정도로 갑작스럽게 나타나고 사라진다.

 

영유아나 학령기 아이들의 경우 증상을 잘 표현하지 못해 체한 것처럼 속이 답답하다고 하거나 보채기만 할 수도 있어 보호자가 만져보고 심장이 너무 빨리 뛰어 응급실로 오기도 한다.

 

두근거림은 수초에서 수 시간까지 지속될 수 있고 증상이 없어진 후 응급실로 내원 하면 심전도에서는 정상으로 보이는 경우가 많아 증상이 있을 때 검사를 하는 것이 중요하다. 

 

진단을 위해 24시간 심전도검사나 심장 전기생리학 검사 등이 필요할 수도 있다. 크게 위험하지는 않으나 오래 지속되는 경우 심박출량 감소로 인한 어지럼증이나 실신 등을 일으킬 수도 있다.

 

차가운 수건을 얼굴에 덮어주거나 숨을 크게 들이마신 후 힘을 주어 참아 복압을 높인 후 내쉬는 발살바호흡 등 응급처치를 해본 후에도 1시간 이상 지속된다면 응급실을 찾는 것이 좋다.

 

심장 이상 외의 원인으로 나타나는 흉통

 

폐질환에 의해서도 흉통이 발생한다. 폐렴이 악화되어 대엽성폐렴이나 흉막염, 흉막삼출 흉수 등에 의해 흉통을 유발할 수 있다. 

 

기침, 가래, 발열 등 감염 증상과 함께 양측이나 우측 등 심장 위치가 아닌 부위에 흉통이 호흡과 동반되어 있다면 심장보다는 호흡기계통 문제를 더 의심할 수 있고 흉부 방사선 촬영으로 확인한다. 

 

특히 흉막삼출이 심해지면 폐나 심장을 압박할 수 있어 주사기로 흡입하거나 배액관을 삽입해야 한다.

 

또 급성장기인 사춘기에 키가 크고 마른 청소년들에서 갑작스러운 호흡곤란과 호흡 시 칼로 찌르는 듯한 흉통이 있다면 기흉이 원인일 수도 있다.

 

심하지 않으면 산소치료만으로도 호전되지만 심한 경우 긴장성기흉으로 발전해 병변 측 폐의 허탈과 심장 및 대정맥의 압박으로 청색증, 호흡곤란, 저혈압으로 실신, 쇼크, 사망 등 치명적인 상태를 유발할 수 있어 즉각적인 응급처치가 필요하다.

 

복부 증상이 흉통으로 나타날 수도 있다. 식전이나 식후에 흉통이 있거나 복부팽만이나 트림, 구역, 구토 등이 동반된다면 심장이나 폐보다는 소화기 원인일 가능성이 크다.

 

 상부 앞가슴뼈 아래쪽 검상돌기나 명치 부위에 식사와 연관된 통증이 있다면 위염이나 급성 장폐색 혹은 심한 변비로 인한 복부팽만을 흉통으로 오인할 수 있다.

 

또 식사 후 신물이 올라오거나 명치부터 가슴이 화끈거리는 작열감이 있는 흉통이라면 위·식도 역류일 가능성이 크다.

 

이러한 소화기 원인에 의한 흉통은 호흡곤란이나 빠른 호흡, 발열 등 전신증상이 거의 없고 제산제나 위장약을 복용하면 증상이 호전되는 경우가 많아 심폐질환과 감별할 수 있다.

 

문헌에 따라 차이가 있지만 보통 소아·청소년에서 흉통으로 가장 많이 응급실을 찾는 경우는 가슴의 근육이나 인대 등에 의한 근골격계가 주요인이다. 근골격계가 원인인 흉통은 주로 수초 정도로 짧고 콕콕 찌르는 듯한 약한 통증이 국한된 부위에 여러 번 반복되고 깊게 숨을 쉬거나 상체를 움직일 때 나타난다. 

 

또 눌렀을 때 통증이 있다. 대개 일시적이고 심하지 않은 염좌나 늑골연골염이 흔하며 최근 무리한 운동이나 활동을 했거나 감기 후 나타나는 경우가 많고 심리적으로 스트레스를 받은 후 발생하기도 하며 1~2주 후 호전된다.

 

전신증상 없이 국소 부위에 압통을 동반한 가벼운 흉통이 진통제 복용으로 증상이 좋아지면 근골격계 흉통일 가능성이 크니 응급실을 찾기보다는 소아·청소년 심장 분과 외래로 방문하는 것을 추천한다.<사진출처:이재창 유방외과>
