우리나라에서도 환자가 가장 많은 만성질환은 고혈압(2021년 기준 1,374만 명)입니다. 국내 20세 이상 성인 인구 10명 중 3명꼴로 고혈압 환자인 셈입니다.

 

젊은 층에서 늘어나는 고혈압

 

 대부분의 사람이 암보다 더 무서워하는 질병인 갑작스런 뇌중풍, 심장병의 가장 중요한 원인은 고혈압입니다.

 

고혈압은 특별한 증상은 없지만, 추운 겨울철에는 혈관조임이 심해 혈압 조절이 더 어려울 수 있어 합병증을 일으키기 쉽습니다. 질병관리청에서 보고한 2007년 대비 2019년 만 30세 이상 성인의 고혈압 유병률은 24.5%에서 27.2%로 증가했습니다. 

 

건강보험심사평가원 자료에 따르면 서울시의 30대 고혈압 환자비율은 2008년 1.8%에서 2018년에는 8.9%(국민건강영양조사 자료)로 높아져 젊은이들의 고혈압 유병률은 급격한 속도로 변화하고 있습니다.

 

 이는 노인들의 질환이었던 고혈압이 이제는 젊은이들까지 위협한다는 것으로, 유전자가 크게 변하지 않았다면 고혈압을 일으킬 수 있는 생활습관 요인의 문제가 더 커지고 있다는 것을 의미합니다.

 

 건강한 식습관, 운동, 금연, 절주 등과 같은 생활습관 교정은 혈압을 떨어뜨리는 효과가 뚜렷하기 때문에 모든 고혈압 환자뿐 아니라 고혈압 전단계 혈압인 분들에게도 적극적으로 권고됩니다. 

 

이러한 좋은 생활습관을 유지하면 고혈압약 한 개 정도의 혈압 강하 효과가 있습니다. 고혈압으로 약물치료 중인 분들도 생활습관 교정을 병행하면 약의 복용량과 갯수를 줄이고, 심뇌혈관질환을 예방할 수 있습니다.

 

20~70대는 혈압, 당뇨, 이상지질혈증 자체를 조절하면 심뇌혈관질환을 예방할 수 있다고 보고됩니다. 70세 이상 고령층에서는 다른 동반질환등의 영향으로 철저한 혈압, 당뇨 조절의 효과가 심뇌혈관질환 발병에 미치는 영향이 크지 않습니다. 

 

또 수축기혈압이 3mmHg, 이완기혈압이 2mmHg만 줄어도, 뇌중풍 23%, 협심증 등 관상동맥 질환은 15%, 사망률은 14% 감소합니다. 이처럼 청년과 중년층에서는 약간의 노력으로 혈압을 조금만 떨어뜨려도 혈관질환을 예방할 수 있습니다.

 고혈압 예방과 관리 방법

 

 ‘침묵의 살인자’로 불리는 고혈압을 예방하고 잘 관리하려면 구체적으로 어떻게 생활해야 할까요?

 

첫째, 혈압에 가장 큰 영향을 미치는 것은 체중입니다. 비만인은 정상인보다 고혈압에 걸릴 위험이 3배 이상 높고, 체중을 4.5kg만 감량해도 혈압이 의미 있게 떨어지고, 일반적으로 10kg 정도 체중을 줄이면 수축기 혈압은 25mmHg, 이완기혈압은 10mmHg 정도 내려갑니다. 특히 60세 이전의 고혈압 환자는 정상 체중을 유지하는 것이 가장 중요합니다.

 

고혈압 예방과 치료를 위한 체중감량 시에는 조금 주의해야 할 부분이 있습니다. 일반적으로 50세 미만인 젊은 연령층의 비만은 고령자의 비만에 비해 제지방량과 골격근량이 많은 것이 특징입니다. 

 

이때 체중감량을 위해 지나치게 저지방식이를 하고, 근육이 빠지는 것을 막기 위해 고단백식이를 하게 되면 몸이 사용할 연료가 적은 스트레스 상황으로 인 식해 교감신경계 활성을 늘리며 혈관과 근육을 조여 혈압이 오르고 고혈압 발생 위험이 증가할 수 있다는 사실입니다. 

 

이와 함께 감정 조절이 어려워지면서 조그마한 자극에도 화를 참기 어려운 상황이 생기기도 합니다. 따라서 끼니를 거르거나 채식만으로 체중을 조절하기보다는 매 끼니 식사량을 조금씩 줄이는 것이 도움이 됩니다.

 

둘째, 고기류 섭취를 조금 줄이고, 대신 올리브유와 생선을 섭취하면 혈압을 낮추는 데 도움이 되는 불포화지방으로 지방 섭취를 전환하게 돼 도움이 됩니다. 

 

동물성 단백질은 근육을 증가시키기도 하고, 포화지방은 혈압을 높일 수 있습니다. 청년층에서 동물성 식품 섭취를 조금 줄이고, 과일, 채소 섭취를 늘리는 것만으로도 수축기혈압을 11mmHg 정도 낮출 수 있으니, 식단 조절을 안 할 이유가 없겠지요?

 

셋째, 규칙적인 운동은 수축기혈압을 4~9mmHg 낮춰줍니다. 주 90~150분 정도의 운동이 적절한데, 하루 30분씩 주 3~5회 약간 빠르게 걷는 운동을 추천합니다. 

 

비만이나 당뇨가 함께 있는 경우는 하루 1시간 정도의 운동을 권유하지만, 단순히 혈압 관리를 목적으로 하는 경우에는 지나친 운동보다는 가벼운 운동이 오히려 도움이 됩니다. 또 체력이 다할 정도의 무리한 근력운동은 갑작스런 근육량 증가와 지방감소를 유발하면서 혈압을 높일 수 있습니다.

나이 듦을 잊지 말고, 같은 일도 조금씩 나누어 하거나 매일 외출하기보다는 미리 외출 스케줄을 잡아 쉬는 날을 두는 것이 혈압으로 인한 심뇌혈관질환 위험을 줄이는 데 크게 도움이 됩니다. 청년층은 체력이 어느 정도 되기 때문에 과로, 과음, 폭식 등 불규칙한 생활습관이 원인인 경우가 많습니다.

 

그 밖에 지나친 음주(남성 소주 3잔, 여성 1~2잔 이상), 흡연, 짜게 먹는 식습관, 기름진 고기, 튀김류 등 나쁜 지방을 많이 섭취하면 혈압이 올라가는 것은 삼척동자도 알 만한 건강 상식입니다.
