【브레이크뉴스 대구】박은정 기자=노인분들이 ‘입맛이 없다’, ‘잠을 잘 못 잔다’, ‘기운이 없다’고 해도 나이 탓 혹은 날씨 탓이려니 하고 대수롭지 않게 여기기 쉽다.

 

하지만 신체 증상을 자주 호소하고 건망증 증상까지 보인다면 노년기 우울증을 의심해봐야 한다. 

 

우울 증상은 65세 이상 인구 10명 중 2~3명이 경험한다고 알려질 정도로 고령층에서 매우 흔한 정신건강 문제다. 노년기 우울증은 다양한 치료법을 통해 회복이 가능하지만 방치할 경우 치매로 이어질 수 있어 더욱 주의해야 한다. 노년기 우울증 증상과 치료법, 치매와 구분하는 방법등을 살펴보자.

 

고혈압은 교감신경에 의한 신경성 요인과 레닌-안지오텐신 기전에 의한 체액성 요인에 의해 발생한다. 여기에 유전, 흡연, 남성, 노령화는 고혈압 유발을 촉진하는 요인이다. 고혈압의 90% 이상은 본태성으로, 원인을 알 수 없는 경우가 대부분이다. 나머지 5~10%는 원인이 명확한 이차성 고혈압에 해당한다. 

 

고혈압의 대부분을 차지하는 본태성 고혈압은 여러 가지 요인이 모여 발생하는데, 이 중에는 유전적인 요인(가족력)이 가장 흔하며, 그 외에 노화, 비만, 짜게 먹는 습관, 운동 부족, 스트레스 등이 있다.

 

 노인 10명 중 3명이 겪는 우울증, 숨은 증상도 많아

 

우울증은 의욕 저하, 우울감을 비롯해 다양한 정신적·신체적 증상을 일으켜 일상 기능의 저하를 가져오는 질환이다. 노년기 우울증이 있는 사람들이 자주 느끼는 대표 증상은 ‘기억력이 나빠졌다’는 것이다.

 

 마치 치매에 걸린 것처럼 인지기능의 문제를 심하게 호소하기도 한다. 이런 경우 진짜 치매는 아니지만 우울 환자에서 치매와 같은 증상이 나타날 수 있다는 뜻에서 ‘가성치매’라고 부르기도 한다. 기분이 가라앉거나 매사에 관심과 의욕이 떨어지는 것도 우울증 환자에게 흔히 나타나는 증상이다. 

 

입맛이 떨어져 체중이 줄거나, 잠들기 어려운 증상도 동반될 수 있다. 몸이 여기저기 아프거나 기운이 없고 소화불량, 가슴 답답함 등의 신체 증상을 자주 호소하거나 건강염려가 과도해 보이는 것도 노년기 우울증의 특징이다.

 

하지만 정작 우울증이 있는 노년층에게 요즘 기분에 대해 물으면 ‘잘 모르겠다’거나 ‘그냥 그렇다’고 대답하는 등 노년기 우울장애 환자들은 자신의 증상을 숨기거나 부정하는 경향이 높다.

 

특히 우리나라 노인들은 자신의 감정 상태에 무관심하거나 직접적으로 표현해본 경험이 적기 때문에 이같은 현상이 두드러질 수도 있다. 따라서 노년층에서는 우울한 기분을 분명하게 호소하지 않더라도 그 이면에 우울증이 숨어 있을 가능성을 반드시 고려해야 한다.

 

우울증, 치매로 이어질 수도

 

노년기 우울증을 잘 진단하고 치료해야 하는 중요한 이유는 또 있다. 바로 치매와 연결될 가능성이 있다는 것. 가성치매로 생각되던 환자에서 우울증상이 호전된 후에도 인지기능 손상이 지속되는 경우도 있기 때문이다. 

 

치매와 우울장애가 공존하는 경우도 흔하다. 치매 환자 중 20~25%가 우울 증상을 호소하기 때문이다. 따라서 인지기능 이상 여부를 꾸준히 관찰해야 한다.

 

노년기 우울증을 앓고 있는 환자들은 크게 두 그룹으로 나뉜다. 첫 번째 그룹은 젊은 나이에 우울증이 발생했다가 노년기에 재발해 나타나는 ‘조발성 우울증’이다. 반면 두 번째 그룹은 젊었을 때 별다른 문제가 없다가 노년기에 처음으로 우울증이 발생하는 경우다. 

 

이를 ‘만발성 우울증’이라고 하는데, 뇌의 퇴행성 변화가 동반됐을 가능성이 높기 때문에 치매로 이어질 수 있어 특히 주의 깊게 경과를 지켜봐야 한다.

 

또 우울증 초기부터 인지기능의 문제가 동반되거나 당뇨, 고혈압 등 뇌신경혈관계에 영향을 주는 신체질환이 있거나, 치료 중 우울 증상은 좋아졌지만 기억에 호전이 없다면 아직 드러나지 않은 신경 퇴행성 질환이 동반됐을 가능성을 고려해야 한다. 우울증 치료에 반응이 좋지 않은 경우에도 이 같은 가능성이 존재하므로 세심한 추적 관찰이 필요하다.

 

가장 확실한 치매 예방법은 ‘노년기 우울증’ 치료

 

우울증이나 치매에 걸리면 일상적인 활동이 줄어들 수 있다. 이럴 때는 우울증으로 인해 의욕이 없고 귀찮아서 ‘안’ 하는 것인지, 아니면 인지기능에 문제가 있어서 실수가 잦고 ‘못’ 하는 것인지를 잘 구분해야 한다.

 

치매는 예방이 매우 중요하다. 치매를 예방하기 위한 중요한 방법 중 하나는 우울증을 잘 치료하는 것이다. 특히 경도인지장애가 있는 사람에게 우울 증상이 있는 경우 치매가 더 빨리 진행되기 때문에 주의해야 한다. 

 

우울감이 지속된다고 느끼면 반드시 전문의를 찾아 상담과 적절한 치료를 받아 치매 진행 가능성도 체크해보는 것이 좋다.

 

이미 우울증으로 치료를 받고 있다면 증상이 호전되더라도 정기적으로 병원에 내원해 인지기능을 체크한다면 건강한 노년을 유지하고 문제를 조기에 발견할 수 있다.

 

노인 우울증은 다양한 방법으로 치료될 수 있어

 

노년층은 우울증 발병률이 높은 데 비해 치료를 받는 비율이 매우 낮다. 우울증을 제대로 관리하지 않으면 삶의 질이 낮아지고 기존의 신체질환이 악화되거나 사망률을 높이는 원인이 되기도 한다.

 

먼저 앓고 있는 신체질환이나 통증, 복용 중인 약물 등이 노년기 우울증의 원인이 될 수 있고 최근에 스트레스를 유발하는 사건이 있었거나 불안정한 환경요인, 경제적 문제 등 다양한 원인이 작용한다는 것을 알아야 한다.

 

이러한 원인들을 포괄적으로 평가하고 개입하면서 적절한 보호자 교육을 병행하는 것 또한 치료 과정에서 중요한 부분이다.

 

노년기 우울증은 약물 치료, 정신 치료, 가족 치료 이외에도 경두개 자기자극법(TMS)이나 경두개 직류자극법(tDCS), 전기경련요법(ECT)등 다양한 방법으로 치료할 수 있다. 대부분 약물 치료를 우선적으로 시행하지만, 치료법 선택에 환자와 보호자의 참여를 유도하는 것이 바람직하다. 

 

항우울제 치료는 노인에서 효능보다는 부작용이 더 중요하기 때문에 성인에 비해 저용량으로 시작하여 치료 용량에 이를 때까지 서서히 증량해야 하며 충분한 기간 동안 사용한 후 반응을 보고 다음 치료를 결정해야 한다.

 

노인들은 대부분 신체질환을 동반하고 있어 다양한 약물을 복용하고 있을 가능성이 높으므로 약물 상호작용도 고려해야 한다. 이후 전문의에게 처방받은 대로 꾸준히 잘 복용한다면 첫 약물 치료에서 40~50%의 환자는 치료 반응을 보이게 된다.

 

노년기 우울증, 치매와 구분하려면

 

우울증과 치매를 구분하기 위해서는 여러 질문을 통한 포괄적인 평가가 이루어져야 한다. 경우에 따라 인지기능 검사나 MRI와 같은 뇌 영상 검사를 시행하기도 한다. 우울증과 치매를 구분할 때 가장 중요한 포인트는 ‘인지기능이 어떻게 나빠져왔는가’에 대한 내용을 확인하는 데 있다.

 

치매를 일으키는 원인 중 80% 이상은 알츠하이머병과 같은 신경 퇴행성질환이다. 이러한 퇴행성 질환은 오랜 시간에 걸쳐 서서히 나빠진다. 

 

예를 들어 우울증 환자의 경우 기억력이 ‘갑자기 나빠졌다’ 혹은 ‘기분 상태에 따라 좋았다 나빴다 한다’고 보고할 수 있지만, 퇴행성 치매 환자는 기억력이 ‘조금씩 점차적으로 더 나빠진다’고 보고한다. 

 

따라서 우울증과 치매를 구분하려면 현재의 인지기능뿐만 아니라 2~3년 전 기억력도 파악해야 한다. 또 작년과 올해의 기억력도 비교해봐야 한다.

 

치매 환자와 우울 환자는 인지기능을 평가할 때 태도에서도 차이를 보인다. 치매 환자는 인지기능 평가에 적극적이지만 오답을 제시하거나, 그런 자신의 모습을 받아들이기가 힘들어 화를 내는 경우가 많다. 하지만 우울환자는 인지기능 평가에 의욕이 적고 모른다는 말을 반복하며 증상을 스스로 과도하게 걱정하며 도움을 청하는 양상을 보인다.
