POSTECH(포항공과대학교)은 생명과학과 및 융합대학원 임신혁 교수 연구팀이 생명과학과 통합과정 이혜나 씨와 함께 자가면역질환과 알레르기 반응을 조절하는 '여포 조절 T세포(Tfr 세포)'의 생성 과정을 규명했다고 밝혔다. 연구 성과는 국제학술지 네이처 커뮤니케이션즈(Nature Communications)​에 게재됐다.

 

자가면역질환은 면역체계가 외부 병원체 대신 자신의 조직을 공격하면서 발생하는 질환으로, 루푸스와 류마티스관절염 등이 대표적인 사례다. 전 세계 인구 약 10%가 경험할 정도로 흔하지만, 현재 치료제는 면역반응을 전반적으로 억제하는 방식이어서 감염 위험을 높이는 한계가 지적돼 왔다.

 

연구팀은 면역계의 균형을 유지하는 핵심 세포인 Tfr 세포에 주목했다. 이 세포는 조절 T세포(Treg)에서 분화해 항체가 과도하게 생성되는 것을 억제하지만, 어떤 신호가 이러한 분화를 유도하는지는 명확히 밝혀지지 않았다.

 

연구 결과 다양한 사이토카인 가운데 인터루킨-6(IL-6)가 Tfr 세포 생성을 가장 강하게 촉진하는 것으로 확인됐다. 이어 IL-6가 세포 내 전사인자인 C/EBPα를 활성화하고, 이 과정에서 Bcl6와 Cxcr5 등 Tfr 세포 형성에 필요한 유전자 발현을 유도해 조절 T세포가 최종적으로 Tfr 세포로 분화하는 과정을 규명했다.

 

동물실험에서도 이 같은 기전은 확인됐다. 연구진이 C/EBPα 유전자를 제거하자 Tfr 세포 형성이 크게 감소했고, 이에 따라 항체 생성이 과도하게 증가하면서 자가면역질환과 알레르기 증상이 악화됐다.

 

환자 혈액 분석에서도 동일한 경향이 나타났다. 루푸스 환자의 조절 T세포에서 C/EBPα 발현 수준이 높을수록 질환 활성도를 나타내는 SLEDAI 지표는 낮게 측정돼, 동물실험에서 확인한 메커니즘이 실제 환자에서도 적용될 수 있음을 뒷받침했다.

 

연구팀은 이번 연구를 통해 IL-6와 C/EBPα로 이어지는 조절 경로가 Tfr 세포 생성의 핵심 축이라는 사실을 처음으로 제시했다고 설명했다. 이 경로를 활용하면 기존처럼 면역 기능 전체를 억제하지 않고 필요한 면역반응만 정밀하게 조절하는 세포치료제와 표적치료제 개발로 이어질 가능성이 있다는 것이다.

 

임신혁 POSTECH 교수는 "IL-6와 C/EBPα를 조절해 Tfr 세포의 생성과 기능을 제어할 수 있다면 자가면역질환과 알레르기를 보다 정밀하게 치료하는 새로운 치료 전략 마련에 도움이 될 것으로 기대한다"며 "향후 알레르기뿐 아니라 암을 포함한 다양한 질환에서 이 기전이 수행하는 역할도 추가로 규명할 계획"이라고 말했다.

 

한편 이번 연구는 POSTECH을 비롯해 이뮤노바이옴, 서울대학교 의과대학, 고려대학교, 아주대학교병원 등 여러 기관이 공동으로 참여해 수행됐다. 연구는 글로벌박사양성사업과 박사과정생 연구장려금 지원사업, 한국연구재단 중견연구(유형2), 바이오미래기술혁신연구센터(B-IRC) 등의 지원을 받아 진행됐다.

POSTECH Research Team First Identifies Operating Principle of ‘Immune Brake’ Suppressing Autoimmune Diseases

 

Domestic researchers have presented new possibilities for the treatment of autoimmune diseases by uncovering a key mechanism that selectively controls excessive immune responses while maintaining normal immune function.

 

POSTECH (Pohang University of Science and Technology) announced that a research team led by Professor Im Shin-hyuk of the Department of Life Sciences and the Graduate School of Convergence Science, in collaboration with Lee Hye-na, a student in the Integrated Program in Life Sciences, has identified the generation process of "follicle-regulating T cells (Tfr cells)," which regulate autoimmune diseases and allergic reactions. The research findings were published in the international journal Nature Communications.

 

Autoimmune diseases are conditions that occur when the immune system attacks its own tissues instead of external pathogens; lupus and rheumatoid arthritis are representative examples. Although they are common enough to affect about 10% of the world's population, current treatments work by suppressing the immune response overall, which has been pointed out as a limitation that increases the risk of infection.

 

The research team focused on Tfr cells, which are key cells that maintain the balance of the immune system. These cells differentiate from regulatory T cells (Treg) to suppress the excessive production of antibodies, but the specific signals that induce this differentiation have not been clearly identified.

 

The study results confirmed that among various cytokines, Interleukin-6 (IL-6) most strongly promotes the generation of Tfr cells. Subsequently, the researchers elucidated the process by which regulatory T cells ultimately differentiate into Tfr cells, revealing that IL-6 activates the intracellular transcription factor C/EBPα, thereby inducing the expression of genes necessary for Tfr cell formation, such as Bcl6 and Cxcr5.

 

This mechanism was also confirmed in animal experiments. When the research team removed the C/EBPα gene, Tfr cell formation decreased significantly; consequently, antibody production increased excessively, exacerbating symptoms of autoimmune diseases and allergies.

 

The same trend was observed in patient blood analysis. In the regulatory T cells of lupus patients, higher levels of C/EBPα expression were associated with lower SLEDAI levels, a measure of disease activity, supporting the applicability of the mechanism identified in animal experiments to actual patients. The research team explained that through this study, they have for the first time demonstrated that the regulatory pathway extending from IL-6 to C/EBPα is a key axis for Tfr cell generation. They suggest that utilizing this pathway could lead to the development of cell therapies and targeted therapies that precisely regulate only the necessary immune responses, rather than suppressing the entire immune function as is currently done.

 

Professor Im Shin-hyuk of POSTECH stated, "If we can control the generation and function of Tfr cells by regulating IL-6 and C/EBPα, we expect this will help in establishing new therapeutic strategies for more precise treatment of autoimmune diseases and allergies." He added, "In the future, we plan to further elucidate the role this mechanism plays in various diseases, including cancer, in addition to allergies."

 

Meanwhile, this research was conducted jointly by several institutions, including POSTECH, Immunobiome, Seoul National University College of Medicine, Korea University, and Ajou University Hospital. The research was conducted with support from the Global PhD Training Program, the PhD Student Research Grant Program, the National Research Foundation of Korea's Mid-Career Researcher Program (Type 2), and the Bio-Innovation Research Center (B-IRC).
