수면패턴의 변화는 '치매의 위험신호'

【한국건강관리협회 】김희진 한양대학교병원 신경과 교수=‘나이가 들면 아침잠이 없어진다’라는 말처럼 노화가 진행되면서 이른 저녁에 잠들고, 아침에는 일찍 일어나게 되는 등 수면 패턴의 변화가 생긴다. 이런 수면 패턴의 변화가 건강에 어떤 영향을 주는지 알아본다.

 

노인 인구의 약 50%가 수면 문제를 호소한다. 장년기 초기에서 노인에 이르기까지 해마다 수면 문제는 증가하고 일생 여성이 남성보다 더 심하다. 불면증은 노인에서 가장 흔한 수면 장애로 60세 이상의 약 40%는 잠이 들기 힘들거나 수면 유지에 어려움을 느끼며, 20% 이상은 심각한 불면증을 겪고 있다. 이외에도 수면 관련 호흡 장애, 주기적 사지운동증, 하지 불안 증후군과 같은 일차적인 수면 질환은 나이가 들면서 증가한다.

 

수면은 평균적으로 3~5단계를 거친다. 각 단계를 60분에서 100분 정도 지속하며 신체 회복에 중요한 역할을 한다. 수면 시간이 하루 6시간 이하로 잠이 부족하거나 10시간 이상으로 잠을 많이 자는 이들이 평균적으로 건강하지 못하다. ‘잠’은 정신적, 육체적 회복과 재충전 에너지 보존, 뇌의 발육, 신경세포의 성숙과 기능을 유지한다. 또한 기억을 정리하게 하고 면역 기능의 회복과 중요 호르몬·체온 조절 기능 등 생존에 매우 중요한 일부 단백질들의 합성과 분해가 수면하는 동안 이루어진다.

 

수면 부족은 우리 인체에 큰 영향을 미친다. 총 50만 명이 넘는 실험 참가자들을 대상으로 한 153개의 논문을 BBC가 검토해 본 결과 수면 부족은 비만, 고혈압, 심혈관 질환, 관상 동맥성 심장 질환, 당뇨 등과 크게 연관 있는 것으로 드러났다.

 

펜실베이니아 대학교 연구에 따르면 하루 수면 시간이 5시간 미만일 경우 고혈압에 걸릴 위험은 5배가 컸으며 5~6시간의 경우 3~5배가 컸다. 당뇨병 위험은 5시간 미만으로 자는 사람이 3배, 5~6시간 자는 사람은 2배가 컸다.

 

특히 불면증을 호소하는 사람들은 대개 깨어있는 시간에 집중력과 기억력이 떨어진다. 실험에서 불면증 환자들은 신속하게 작업을 전환하거나 시각 정보를 기억하는 데 어려움을 겪었다. 따라서 불면증은 치매나 인지기능 저하의 위험요인이 된다. 그렇다면 어떤 불면증의 유형이 치매와 관련이 있을까?

 

◇수면 유지 장애

 

노년기 불면증은 자는 동안 자주 깨서 다시 잠들기 힘들거나 깊이 잠들지 못하는 ‘수면 유지 장애’가 많다. 최근 연구에 따르면 수면의 질이 치매의 발병에도 영향을 미치는 것으로 나타났다. 알츠하이머 치매는 뇌 신경세포에 베타-아밀로이드라는 단백질이 비정상적으로 쌓이면서 뇌 신경세포의 기능 장애를 일으키며 발병한다.

 

깨어 있는 동안 뇌가 활동하면서 발생한 베타-아밀로이드는 밤에 깊은 잠을 자는 동안 배출되는데 숙면하지 못하면 뇌에 베타-아밀로이드가 축적돼 알츠하이머 치매 같은 퇴행성 질환이 발병한다.

 

◇ 수면 호흡 장애

 

수면 호흡 장애(코골이)는 잠자는 동안 호흡 기류가 정지하거나 현저히 감소하는 현상이 반복되는 것을 특징으로 한다. 가장 강력한 위험 요인은 비만이며 알코올, 진정제, 수면박탈, 비강 울혈 그리고 반듯이 누운 수면 자세 등에 의해 악화된다.

 

뇌가 야간에는 지속적인 저산소증에 빠지기 때문에 환자들은 졸음으로 인해 운전 중에 잠이 드는 경우도 있다. 아침에 머리가 무겁고 민첩성, 집중력, 기억력, 판단력이 저하되며, 예민·불안·우울한 성격으로 변할 수 있다. 삶의 질이 저하되고 인지 기능 저하, 고혈압, 부정맥, 심부전증, 뇌졸중의 위험이 증가한다.

 

◇ 하지 불안 증후군과 주기적 사지운동증

 

하지 불안 증후군은 가만히 앉아 있거나 자려고 누워있을 때 다리에 불편함이 느껴지고 벌레가 기어 다니는 듯한 이상한 느낌이 들어 다리를 움직이고 싶은 강한 충동이 생기는 것을 말한다. 가만히 있으면 증상이 악화되고 사지를 움직여야 증상이 완화되기 때문에 잠들 수 없게 되어 낮 동안에 비정상적으로 피곤하고 졸리게 되어 일이나 사회생활에 지장을 받게 된다.

 

저녁이나 밤에만 이런 증상이 있거나 낮과 비교해 저녁이나 밤에 하지의 증상이 악화되는 특징이 있다. 원인은 밝혀지지 않았으나 이 질환을 일으킨다고 알려진 질환들이 역시 인지기능 저하와 관련이 있기 때문에 고령자에게 발생할 경우 확인이 필요하다.   

 

◇ 렘수면 행동 장애

 

렘수면 행동 장애는 렘수면 동안 골격근이 전혀 활동하면 안 되는데, 이에 문제가 발생하여 꿈의 내용을 행동화하는 수면 질환으로 노인에서 호발한다. 퇴행성 신경 질환에서 맨 처음 나타나는 특징 중 하나로 알려져 있다. 특히 렘수면 행동 장애를 진단받은 환자의 50%에서 3~4년 이내에 파킨슨병이나 다계통위축(multiple system atrophy)이 발생하였다는 연구 결과가 있다.

 

또한 렘수면 행동 장애를 일으키는 단백질과 치매, 파킨슨병을 일으키는 병적인 단백질이 동일하기 때문에 이런 증상을 보인다면 반드시 신경과 전문의 진료를 받을 필요가 있다. 치매와 신경퇴행성 질환을 일으키는 단백질은 증상이 나타나기 몇 년 전부터 뇌에 축적된다.

 

따라서 깊은 잠을 자지 못하는 수면 장애가 있는 경우 적절한 치료와 운동과 식이조절을 통한 생활습관 개선으로 숙면하도록 노력하는 것이 중요하다.

◇ Sleep maintenance disorder

Recent studies have shown that sleep quality also affects the onset of dementia. Alzheimer's dementia is caused by abnormal accumulation of a protein called beta-amyloid in brain neurons, causing dysfunction of brain neurons.


Beta-amyloid generated while the brain is active while awake is released during deep sleep at night. If you fail to sleep, beta-amyloid accumulates in the brain, leading to degenerative diseases such as Alzheimer's dementia.

 

◇ sleep breathing disorder

 

Sleep breathing disorder (snoring) is characterized by repeated symptoms of stopping or remarkably reducing respiratory airflow while sleeping. The strongest risk factor is obesity, which is exacerbated by alcohol, sedatives, sleep deprivation, nasal congestion, and a supine sleeping position.


Patients sometimes fall asleep while driving due to drowsiness, as the brain suffers from persistent hypoxia at night. Your head is heavy in the morning, your agility, concentration, memory, and judgment are deteriorated, and you can turn into a sensitive, anxious, and depressed personality. Quality of life decreases, cognitive decline, high blood pressure, arrhythmia, heart failure, and an increased risk of stroke.

 

◇ Restless leg syndrome and periodic limb movement syndrome

 

Restless legs syndrome is when you are sitting still or lying down to sleep, you feel discomfort in your legs and a strange sensation like a bug crawling, resulting in a strong urge to move your legs. If you stay still, your symptoms worsen and your limbs move to relieve symptoms, so you can't fall asleep, and you'll be abnormally tired and drowsy during the day, which will interfere with your work or social life.


This symptom is present only in the evening or at night, or symptoms of the lower extremities worsen in the evening or at night compared to the daytime. Although the cause has not been identified, diseases known to cause this disease are also related to cognitive decline, so if it occurs in the elderly, it is necessary to confirm it.

 

◇ REM sleep behavior disorder

 

REM sleep behavior disorder is a sleep disorder that makes the content of dreams behavioral because skeletal muscles should not be active during REM sleep at all. It is known as one of the first features of neurodegenerative diseases. In particular, studies have shown that 50% of patients diagnosed with REM sleep behavior disorder developed Parkinson's disease or multiple system atrophy within 3 to 4 years.


Also, since the protein that causes REM sleep behavior disorder and the pathological protein that causes dementia and Parkinson's disease are the same, it is necessary to consult a neurologist if you have these symptoms.
