최근 나이와 상관 없이 허리 통증을 호소하는 사람들이 많아지고 있다. 갑자기 찾아오는 ‘찌릿찌릿’한 허리 통증을 방치하면 걷기 힘들 정도로 다리가 저려오거나 심하면 마비 또는 대소변장애까지 일으킬 수 있다. 이러한 증상을 일으키는 척추질환 ‘허리 디스크’와 ‘척추관 협착증’에 대해 알아보자.

 

 

 

대표적인 척추 질환

 

척추 질환은 연령이 증가하면서 척추에서 발생하는 전형적인 퇴행성 병변이다. 척주의 대표적인 구조물은 척추체, 추간판, 인대 그리고 후관절을 비롯한 다양한 뼈로 이루어진다. 추간판은 척추체 사이에서 움직임을 보존해 주고 충격을 완화하는 역할을 하는 조직으로 정상적으로 척추체 사이에 있어야 하는 조직이다.

 

후관절은 여러 척추체를 연결해 주는 역할을 하고 황색 인대를 비롯한 다양한 인대는 척추의 안정성을 유지해 주는 역할을 하게 된다. 이러한 구조물은 신경이 지나가는 척추관을 형성하여 신경을 보호하게 된다. 척추관을 구성하는 구조물은 연령이 증가하면서 다양한 변화가 발생하게 되는데 이로 인한 대표적인 질환으로 추간판 탈출증과 척추관 협착증이 있다.

 

추간판 탈출증과 척추관 협착증의 증상

‘추간판 탈출증’은 연령이 증가하면서 수핵의 수분이 빠져나가면서 푸석해지고 어떠한 압력의 증가로 인하여 추간판이 빠져나와 신경을 압박하는 질환이다. 추간판 탈출증은 대부분 ‘요추 추간판 탈출증’과 ‘경추 추간판 탈출증’이다. 우리가 흔히 ‘허리 디스크’ ‘목 디스크’라고 부르는 질병이다.

 

또 다른 척추 질환인 ‘척추관 협착증’은 추간판을 비롯하여 후관절과 황색 인대 등이 비후되어 척주관을 좁혀 신경을 압박하는 질환이다.추간판 탈출증의 대표적인 증상은 허리 통증이지만 모든 환자가 허리에 통증을 느끼는 것은 아니다. 탈출한 디스크가 신경을 누르느냐 아니냐에 따라 통증 유무와 정도가 달라지기 때문이다.허리를 중심으로 한 요추 부위의 추간판이 탈출하면 다리의 찌릿함과 허리 통증, 다리의 감각 이상, 좌골신경통 등이 증상으로 나타난다.

 

좌골신경통은 엉덩이와 허벅지 뒤쪽, 다리까지 이어지는 통증을 말한다. 경추 추간판이 탈출하면 목과 어깨, 위팔, 손, 손가락 등에서 통증을 느낄 수 있다. 팔의 근력이 약해지고 기침을 하거나 웃을 때 통증이 심해지는 것이 특징이다.두 질환 모두 하지로 내려가는 신경을 압박하여 다양한 신경학적인 증상을 유발한다.

 

대표적인 증상인 하지 통증 또는 저립감은 압박되는 신경의 분포에 따라 나타나게 되며, 심각한 압박으로 신경이 손상을 받게 된다면 감각 저하나 마비 등이 발생할 수 있다. 더 심한 경우에는 대소변 장애까지 발생하는 ‘마미증후군’도 발생할 수 있다.척추관 협착증의 경우 일반적인 추간판 탈출증과 구별되는 증상은 보행 시에 하지 통증이 더 심해지고 쭈그려 앉거나 쉬면 통증이 호전되는 신경학적인 간헐적 파행이 특징적이다.혈관성 파행도 있는데 이는 보행 시나 쉬는 자세에서도 증상이 호전되지 않는 것으로 하지의 혈관 질환으로 인하여 발생하게 된다.

 

척추 질환의 진단과 치료

 

정확한 진단은 신경학적인 증상을 기본으로 하여 MRI를 비롯한 CT를 통한 정밀 검사로 확인해야 한다. CT는 뼈 구조를 보기 좋은 검사이며 MRI는 신경 및 연부 조직과 추간판을 비롯한 근육 등의 변성도 확인이 가능한 검사로 차이가 있다. 비록 고가의 검사이기는 하지만 MRI 검사는 정확한 판단을 위하여 필요한 검사이다.

 

두 질환의 근본적인 치료 방법은 수술적인 제거이다.하지만 척추질환 자체로 인하여 죽고 사는 문제가 발생하는 것은 아니므로 감내할 수 있는 정도의 통증이며 일상생활에 제약이 불편한 정도로 느끼지 않는다면 보존적인 치료로 증상을 유지하는 것이 치료의 시작이 된다. 즉, 반드시 수술이 필요한 마비 등의 신경학적인 결손이 발생한 경우를 제외하고는 수술이 선행될 필요는 없다.

 

보존적인 치료의 일반적인 순서는 안정가료를 비롯한 약물치료와 물리 치료 등이 있을 수 있고 증상의 호전이 없다면 신경 차단술을 비롯한 다양한 시술이 도움이 될 수 있다.필자의 경우는 고가의 시술보다는 신경 차단술을 선호하는데 그 사유는 아직까지는 시술이 신경 차단술에 비해서 월등한 효과가 있다고 보기 어렵기 때문이다. 이러한 시술 후에도 증상의 호전이 없다면 종국에는 수술적인 치료가 요구된다.

 

추간판 탈출증의 경우는 내시경을 이용하거나 현미경을 이용하여 신경을 보호하는 뼈 구조물을 일부 제거하고 탈출된 추간판 조각을 제거하여 신경을 풀어주는 수술을 시행한다. 대부분은 5cm미만의 절개로 가능하다. 척추관 협착증의 경우는 신경을 압박하는 뼈와 인대를 비롯한 다양한 구조물을 제거하여야 하는데 광범위한 절개가 필요하고 척추의 불안정증이 유발될 가능성이 있다면 나사못을 이용한 고정이 필요할 수도 있다.

 

척추 건강을 지키는 법

 

척추 질환은 나이가 들면서 발생하는 퇴행성 질환이므로 뚜렷한 예방법은 없다. 다만 질환을 지연시키기 위한 방법으로 가장 중요한 것은 올바른 자세와 스트레칭이다. 요추는 정상적으로 배가 나오는 C자 형태를 가지고 있으며 이는 각각의 추간판에 걸리는 압력을 분산시키는 효과가 있다. 몸을 굽히는 동작이나 자세는 추간판 질환을 유발할 가능성이 있게 된다.

 

따라서 앉거나 보행 시 허리를 펴는 자세가 중요하다. 또한 이러한 자세는 바닥 생활을 할 때 저하될 수 있어 의자와 침대를 이용한 생활도 필요하게 된다. 복근이나 허리 주변 근육을 꾸준히 강화하는 것도 중요하다. 척추를 잡아주는 허리 근육이 강해지면 척추의 퇴행을 막는 효과가 있기 때문이다.빠르게 걷기, 수영, 등산, 간단한 에어로빅 등을 통해 허리 근육을 강화하는 운동을 하는 것이 효과적이다.

 

다만 통증이 있는 상태에서는 무리한 운동은 권고하지 않으나 통증이 호전되는 양상이라면 점차 운동량을 늘려 근육 운동 등을 권한다.현대인의 허리 통증과 척추 질환의 이유는 앉아 있는 시간이 많고 편안함만을 추구하는 자세 때문이다. 오래 앉아 있는 것은 서 있거나 누워 있을 때보다 척추에 가해지는 압력이 훨씬 크다. 시간당 1회 정도는 자리에서 일어나 주변을 돌며 몸의 긴장감을 없애 주거나 틈틈이 스트레칭을 통해 굳은 척추 근육과 인대를 풀어주는 것도 필요하다.

'Intervertebral disc herniation' is a disease in which the water in the nucleus is dehydrated as age increases, and the intervertebral disc comes out due to an increase in pressure and compresses the nerve. Most of the herniated discs are'lumbar herniation' and'cervical disc herniation'. It is a disease that we commonly refer to as a'waist disc' or'neck disc'. Another spinal disease,'spine canal stenosis', is a disease that compresses nerves by narrowing the spinal canal due to thickening of the intervertebral discs, the facet joints, and the yellow ligaments.

 

The typical symptom of herniated intervertebral disc is back pain, but not all patients have back pain. This is because the presence and degree of pain vary depending on whether the prolapsed disk presses the nerve.

When the intervertebral disc in the lumbar region, centered on the lower back, prolapses, symptoms include tingling in the legs, back pain, numbness in the legs, and sciatic nerve pain. Sciatica is a pain that extends to the buttocks, the back of the thigh, and the legs. When the cervical disc is prolapsed, pain can be felt in the neck, shoulders, upper arms, hands, and fingers. It is characterized by weak arm strength and intensifying pain when coughing or laughing.

 Both diseases cause a variety of neurological symptoms by compressing the nerves going down to the lower extremities. A typical symptom, lower extremity pain or numbness, appears according to the distribution of the nerve to be compressed, and if the nerve is damaged by severe compression, sensation or paralysis may occur. In more severe cases, ‘Mami syndrome’, which even causes stool disorder, can also develop.

 In the case of spinal canal stenosis, a symptom that is distinguished from general disc herniation is characterized by intermittent neurological claudication, in which pain in the lower extremities becomes more severe when walking and the pain improves when squatting or resting.

There is also vascular claudication, which is caused by vascular disease in the lower extremities, as symptoms do not improve even when walking or in a resting position.

 

Diagnosis and treatment of spinal disorders

Accurate diagnosis should be confirmed by detailed examination through CT including MRI based on neurological symptoms. CT is a good test to see the bone structure, and MRI is a test that can also check the degeneration of nerves and soft tissues and muscles, including the intervertebral disc, and there is a difference. Although it is an expensive test, the MRI test is a necessary test for accurate judgment. The fundamental treatment for both diseases is surgical removal.

 However, since the problem of dying and living does not occur due to the spinal disease itself, it is a pain that can be tolerated, and if you do not feel the constraints in your daily life to an uncomfortable degree, maintaining the symptoms with conservative treatment begins the treatment. In other words, surgery does not need to be preceded unless a neurological defect such as paralysis, which requires surgery, has occurred. The general sequence of conservative treatment may include stable treatment, medication, physical therapy, etc. If there is no improvement in symptoms, various procedures including nerve block may be helpful.

 In my case, I prefer a nerve block rather than an expensive procedure because it is difficult to see that the procedure has a superior effect compared to a nerve block so far. If symptoms do not improve even after these procedures, surgical treatment is required in the end.

In the case of herniated intervertebral disc, surgery is performed to loosen the nerve by removing part of the bone structure that protects the nerve using an endoscope or a microscope, and removing a piece of the prolapsed intervertebral disc. Most can be made with an incision less than 5cm. In the case of spinal canal stenosis, various structures including bones and ligaments that compress nerves must be removed. If extensive incisions are required and there is a possibility that spinal instability may be caused, fixation using screws may be necessary.

 

How to keep your spine healthy

Spinal disease is a degenerative disease that occurs with age, so there is no clear prevention method. However, the most important way to delay the disease is correct posture and stretching. The lumbar spine has a C-shape from which the abdomen normally appears, and this has the effect of dispersing the pressure applied to each intervertebral disc. Any movement or posture that is bending over has the potential to cause intervertebral disc disease.

 Therefore, it is important to straighten your back when sitting or walking. In addition, this posture can be deteriorated when living on the floor, so it is necessary to use a chair and a bed. It is also important to steadily strengthen your abs or the muscles around your lower back. This is because when the back muscles that hold the spine become stronger, it has the effect of preventing degeneration of the spine.

 It is effective to exercise to strengthen your back muscles through fast walking, swimming, mountain climbing, and simple aerobics. However, excessive exercise is not recommended in the state of pain, but if the pain is improving, gradually increase the amount of exercise to recommend muscle exercise.

The reason for back pain and spinal disease in modern people is that they have a lot of sitting time and a posture that seeks only comfort. Sitting for a long time puts much more pressure on your spine than when standing or lying down. It is also necessary to get up from a seat once an hour to get rid of the tension in the body by turning around, or to loosen the stiff spinal muscles and ligaments through stretching.
