원인도 증상도 다양한 노인성 배뇨장애

【브레이크뉴스 대구】박은정 기자=나이가 들어감에 따라 신체에는 많은 변화가 나타난다. 비뇨기계에 포함된 장기 또한 마찬가지여서 전립선은 점점 커지고, 방광 기능은 점차 떨어져서 배뇨장애가 나타난다.

 

실제로 소변을 자주 본다든지, 소변 줄기가 약한 증상 등 다양한 배뇨장애 증상을 호소하며 많은 노인 환자가 비뇨의학과 외래를 방문한다.

 

노인들이 겪는 배뇨장애 증상은 다양하다. 소변보는 간격이 2시간 이하이거나 하루 8회 이상 소변을 보는 빈뇨, 수면 중에 요의를 느껴서 깨는 야간뇨, 소변이 갑자기 마려우면서 참기 어려운 절박뇨, 소변 줄기가 약한 세뇨, 시작할 때 시간이 오래 걸리는 지연뇨, 소변이 중간에 끊기는 단축뇨 및 소변을 본 후에 개운치 않게 남아 있는 잔뇨 등이다.

 

소변보는 과정은 크게 저장 과정, 배뇨 과정으로 나눌 수 있는데, 저장 과정은 신장에서 만들어진 소변이 방광으로 들어오면서 방광이 서서히 차는 과정이다. 

 

평균적으로 소변이 300~400cc 정도 차면 뇌교에 있는 배뇨중추에 소변이 찼다는 신호가 온다. 그러면 배뇨중추에서는 배뇨를 위한 신호를 보내고 이 신호를 받은 요도 괄약근이 이완되고 방광 근육이 수축하면서 배뇨 과정이 시작된다. 

 

소변을 다 본 뒤에는 요도가 수축하고 방광이 이완되면서 다시 저장 과정이 시작된다. 이러한 저장 과정에 문제가 있으면 빈뇨, 야간뇨나 절박뇨와 같은 증상이 생기고, 배뇨 과정에 문제가 있으면 세뇨, 지연뇨, 단축뇨 및 잔뇨감 등의 증상이 나타난다.

 

배뇨장애는 일상생활을 영위하는 데 불편함을 유발하는 경우가 많다.

 

빈뇨나 급박뇨가 심한 경우에는 야외 활동에 제약을 받을 수 있고, 소변 줄기가 약하고 소변보는 데에 시간이 많이 걸리는 경우에는 그 자체로 스트레스를 받기도 한다.

 

남성 노인의 방광 건강을 위협하는 전립선비대증

 

남성 노인에게 생기는 배뇨장애의 대표적인 원인 중 하나가 전립선비대증이다. 전립선은 방광 경부와 요도를 둘러싸고 있는 20cc 내외의 크기를 가진 장기로, 나이가 들어감에 따라서 점차 커진다. 

 

전립선이 커지면 전립선 요도를 압박하여 방광출구폐색을 유발한다. 이러한 이유로 전립선비대증 초기에는 세뇨, 지연뇨, 잔뇨 증상이 주로 나타난다. 전립선비대가 지속되면 방광출구폐색에 대한 보상 작용으로 방광근이 점차 두꺼워지면서 방광이 점차 딱딱해져 유순도가 낮아진다. 그 결과 방광 용적이 점차 줄어들고 빈뇨, 급박뇨와 같은 저장 증상까지 나타나기도 한다.

 

전립선비대를 진단하기 위해서는 일반적으로 배뇨 증상과 관련된 설문지, 소변검사, 전립선특이항원을 포함한 혈액검사, 전립선 초음파검사 및 요속·잔뇨 검사를 시행한다. 

 

소변검사, 혈액검사에서 배뇨 증상을 유발할 수 있는 염증성 질환 등이 동반되지 않았는지 확인하고 전립선특이항원은 전립선암 여부를 확인하는 선별검사로 이용된다. 전립선 초음파 검사로는 전립선 크기를 측정하고, 요속·잔뇨 검사에서는 소변 줄기가 어떤지, 소변량과 잔뇨량이 얼마나 되는지 객관적으로 측정할 수 있다.

 

전립선비대증으로 진단되면 검사 결과를 바탕으로 다양한 약제를 이용한 약물치료가 우선적으로 이루어진다. 알파차단제는 전립선 요도의 압력과 긴장을 낮춰 배뇨 증상을 개선해준다. 알파차단제는 심한 부작용 없이 빠른 효과를 나타내기 때문에 가장 우선적으로 사용되는 약제이다.

 

일부 환자에서는 눕거나 앉았다가 일어날 때 어지러운 증상이 나타나는 기립성저혈압, 정액이 나오지 않는 역행성 사정과 같은 부작용이 나타날 수는 있다. 

 

안드로겐 억제제는 남성호르몬인 디하이드로테스토스테론을 억제해 전립선 크기를 줄이는 작용을 하며 일반적으로 전립선이 30cc 이상인 경우에 사용이 권고된다. 남성호르몬을 억제하는 만큼 성기능이 감소되는 부작용이 있다. 그 외에 저장 증상이 심한 경우에는 과민성 방광에서 주로 사용되는 항콜린제를 사용해볼 수도 있다.

 

수술적 치료로는 경요도 전립선절제술, 레이저수술 및 전립선결찰술 등이 있다. 경요도 전립선절제술은 요도를 통해 내시경을 넣어서 커진 전립선을 내부에서 깎아내는 수술로, 수십 년 전부터 시행해온 전립선비대증의 표준 수술 방법이다. 수술 후에는 배뇨 증상이 획기적으로 개선되지만, 역행성 사정과 같은 합병증이 나타날 수 있다. 

 

레이저수술은 홀뮴레이저나 KTP 레이저를 이용한 수술이 있으며, 일반적으로 경요도 전립선절제술에 비해 출혈과 같은 합병증이 좀 더 적은 것으로 알려져 있다. 전립선결찰술은 전신마취 없이 당일 시술이 가능하며 전립선 크기가 많이 크지 않은 환자군에서는 증상 개선에 효과가 있다. 

 

하지만 전반적으로 다른 수술 방법에 비해서는 효과가 상대적으로 떨어지는 것으로 알려져 있으며, 건강보험이 적용되지 않아 비용이 많이 든다는 단점이 있다.

 

 

여성 노인은 과민성방광이 가장 흔한 원인

 

여성에게 가장 흔한 배뇨장애 원인은 과민성방광이다. 국제요실금학회에서 정의하는 과민성방광은 요로감염이 없는 상태에서 절박뇨가 있으면서 빈뇨와 야간뇨가 동반된 경우이다. 일반적으로 소변을 자주 보거나 급한 경우가 많으며, 증상이 심한 경우에는 절박성요실금이 동반되기도 한다.

 

과민성방광 환자에게도 설문조사와 소변검사, 요속·잔뇨 검사 등이 시행되며 치료에 반응이 없거나 증상이 심할 경우에는 요역동학 검사를 실시해 방광 기능을 자세하게 평가하기도 한다.

 

과민성방광을 치료할 때 가장 먼저 권고되는 것은 행동치료로, 배뇨 습관 중에서 올바르지 못한 부분을 개선하는 것이다. 과민성방광 환자들은 한 번에 나오는 소변량이 적어서 200cc가 채 안 되는 경우가 많다.

 

 이런 경우에는 소변 참기 훈련이 도움이 될 수 있다. 처음 소변이 마려울 때 어느 정도 시간이 지나고 나면 소변 마려운 느낌이 없어지는 순간이 온다. 그러다 두 번째로 소변 마려운 증상이 있을 때 배뇨를 하는 것이 대표적인 방광 훈련의 예이다.

 

약물치료로는 항콜린제를 사용할 수 있다. 이는 부교감신경에 작용해 소변이 찼다는 신호를 배뇨중추로 보내는 과정을 억제하여 빈뇨나 급박뇨증상을 호전시킨다. 최근에는 β-3 교감신경 수용체에 작용하여 항콜린제와 비슷한 효과를 내는 β-3 작용제도 많이 사용된다.

 

약물치료에 반응이 없는 경우에는 방광 내 보톡스 주입술이나 천추신경 조정술과 같은 수술적 치료를 시도해볼 수 있다.

 

요실금도 흔하게 발생하는 질환으로, 절박성요실금과 복압성요실금으로 나뉜다. 절박성요실금은 위에 언급했듯이 과민성방광으로 인하여 나타나는 증상이며 주된 치료법은 약물치료이다. 

 

복압성요실금이 흔히들 이야기하는 요실금으로, 기침하거나 뛸 때와 같이 복압이 올라가는 상황에서 소변이 새는 증상이다. 이는 골반 근육이 약해져서 방광을 짓누르는 압력을 견디지 못하여 요누출이 발생하며 임신과 출산, 폐경, 골반장기 탈출 등이 원인일 수 있다.

 

비수술적 치료로는 약물치료 외에 골반저근운동(케겔 운동), 바이오피드백이나 전기자극요법 등이 있으나 치료 효과가 나타나는 데 시간이 오래 걸리고 효과가 부족한 경우도 많다. 

 

수술적 치료로는 슬링 수술을 많이 하는데, 이는 중부 요도 아래에 인공테이프를 걸어서 요도를 전방으로 올려주는 수술을 말한다. 수술을 하기 전에는 요역동학검사가 필수이며 방광의 과민성이나 요누출압(어느 정도의 압력이 가해졌을 때 소변이 새는지)을 확인한다.

 

 배뇨장애를 유발하는 다양한 원인

 

그 외에도 배뇨장애를 유발하는 다양한 원인이 있다. 여성에서는 급성방광염이 배뇨장애의 흔한 원인인데, 이는 방광에 세균이 침입하여 생기는 단순 감염이다. 

 

배뇨 시 느끼는 요도의 통증이 가장 흔한 증상이며, 혈뇨가 동반되는 경우도 많다. 증상으로는 빈뇨와 야간뇨, 절박뇨 및 잔뇨감도 흔하게 동반된다. 소변검사로 진단하며, 일반적인 단순 방광염의 경우에는 항생제 투여로 쉽게 치료된다.

 

좀 더 드물게는 방광의 질환으로 인하여 배뇨장애가 나타나기도 한다. 방광결석이 있을 때는 결석이 방광을 자극함으로써 과민성방광과 비슷한 증상이 나타나기도 하며 때에 따라서는 배뇨통이 동반되기도 한다.

 

크기가 큰 방광결석은 내시경과 레이저를 이용하여 결석을 파쇄한 후에 제거하는 수술을 해야 한다.

 

방광암이 있을 때도 급박뇨와 같은 증상들이 나타날 수 있으며 소변검사나 내시경, 복부 CT와 같은 검사들을 실시해서 진단한다. 그리고 대부분의 경우에는 수술적 치료가 필요하다.

 

일반적으로 배뇨장애는 일상생활을 영위하는 데 불편함을 유발하는 경우가 많다. 빈뇨나 급박뇨가 심한 경우에는 야외 활동에 제약을 받을 수 있고, 소변 줄기가 약하고 소변보는 데 시간이 많이 걸리는 경우에는 그 자체로 스트레스를 받기도 한다. 

 

야간뇨로 인해 수면 부족에 시달리기도 하는 등 일상생활에 어려움을 겪기도 한다. 이처럼 배뇨장애의 원인이 워낙 많고 치료 방법도 다양하기 때문에 배뇨장애 때문에 고통받고 있다면 비뇨의학과 진료를 통해 적절한 검사와 치료를 받을 필요가 있다.

BPH threatens bladder health in older men

 

An enlarged prostate is one of the most common causes of urination problems in the elderly. The prostate is an organ with a size of about 20cc that surrounds the bladder neck and urethra, and it gradually grows with age.

 

When the prostate enlarges, it compresses the prostatic urethra and causes bladder outlet obstruction. For this reason, precipitation, delayed urine, and residual urine symptoms mainly appear in the early stages of BPH. If the enlarged prostate continues, the bladder muscle gradually thickens as a compensatory action for bladder outlet obstruction, and the bladder becomes harder and less flexible. As a result, bladder volume gradually decreases, and storage symptoms such as frequent urination and urgency may appear.

 

In order to diagnose an enlarged prostate, questionnaires related to urination symptoms, urinalysis, blood tests including prostate-specific antigen, prostate ultrasound, and urine flow and residual urine tests are generally performed.

 

Urine and blood tests are used to check whether inflammatory diseases that can cause urination symptoms are not present, and prostate-specific antigen is used as a screening test to check for prostate cancer. Prostate ultrasound can measure the size of the prostate, and urine flow and residual urine test can objectively measure what kind of urine stream and how much urine and residual urine are.

 

When diagnosed with benign prostatic hyperplasia, drug treatment using various drugs is prioritized based on the test results. Alpha blockers improve urination symptoms by lowering pressure and tension in the prostate urethra. Alpha blockers are the most preferred drugs because they show a rapid effect without severe side effects.

 

In some patients, side effects such as orthostatic hypotension, which causes dizziness when getting up from lying down or sitting, and retrograde ejaculation in which semen does not come out, may occur.

There is a side effect of decreasing sexual function as much as suppressing male hormones. Surgical treatments include transurethral prostatectomy, laser surgery, and prostate ligation. Transurethral prostatectomy is a surgical procedure in which an enlarged prostate is removed from the inside by inserting an endoscope through the urethra. Although urination symptoms are dramatically improved after surgery, complications such as retrograde ejaculation may occur.

 

Laser surgery includes surgery using holmium laser or KTP laser, and is known to have fewer complications such as bleeding compared to transurethral prostatectomy in general. Prostate ligation can be performed on the same day without general anesthesia, and it is effective in improving symptoms in patients with not very large prostates.

The most common cause of overactive bladder in older women

 

The most common cause of dysuria in women is overactive bladder. Overactive bladder, as defined by the International Association for Urinary Incontinence, is a case of urgency and nocturia accompanied by frequency and nocturia in the absence of urinary tract infection. In general, frequent or urgent urination is common, and if the symptoms are severe, urge incontinence may be accompanied.

 

Patients with overactive bladder are also given a questionnaire, urinalysis, and urine rate and residual urine test.

 

When treating overactive bladder, the first recommendation is behavioral therapy, which improves the incorrect part of urination habits. Patients with overactive bladder often have less than 200cc of urine output at a time.

 

 In this case, urination training can help. When you first need to urinate, after a certain amount of time has passed, there comes a moment when the urge to urinate disappears. Then, urinating when there is a second urge to urinate is a typical example of bladder training.

 

Anticholinergics may be used for drug treatment. It acts on the parasympathetic nerve to suppress the process of sending a signal that the urine is full to the urination center, thereby improving the symptoms of frequent urination or urgency. Recently, β-3 agonists, which act on β-3 sympathetic receptors and produce similar effects to anticholinergics, are also widely used.

 

If there is no response to drug treatment, surgical treatment such as intravesical botox injection or sacral nerve manipulation may be attempted.

 

Urinary incontinence is also a common disease and is divided into urge incontinence and stress incontinence. As mentioned above, urge incontinence is a symptom caused by overactive bladder, and the main treatment is medication.

 

Stress incontinence is a commonly talked about urinary incontinence, and it is a symptom of leakage of urine when the abdominal pressure rises, such as when coughing or running. This is because the pelvic muscles are weak and cannot withstand the pressure on the bladder, which causes urine leakage.

 

Non-surgical treatments include pelvic floor muscle exercise (Kegel exercise), biofeedback, and electrical stimulation in addition to drug treatment, but the treatment takes a long time to appear and the effect is often insufficient.

 

Sling surgery is often used as a surgical treatment, which refers to an operation in which an artificial tape is placed under the central urethra to raise the urethra forward. Before surgery, urodynamics test is essential, and bladder hypersensitivity or leaky pressure (how much pressure is applied to see if urine leaks) is checked.

 

 Various causes of urination problems

 

There are many other causes of urination problems. In women, acute cystitis is a common cause of dysuria, which is a simple infection caused by bacterial invasion of the bladder.

 

Urethral pain felt during urination is the most common symptom and is often accompanied by hematuria. Symptoms include frequent urination, nocturia, urgency and residual urination. Diagnosed by urinalysis, and general simple cystitis can be easily treated with antibiotics.

 

More rarely, bladder disorders may cause urination problems. When bladder stones are present, the stones irritate the bladder, causing symptoms similar to overactive bladder, and sometimes accompanied by dysuria.

 

Large bladder stones require surgery to break them up using an endoscope and laser and then remove them.

 

Symptoms such as urgency may also appear when bladder cancer is present, and it is diagnosed by performing tests such as urinalysis, endoscopy, and abdominal CT. And in most cases, surgical treatment is required.

 

In general, urination disorders often cause discomfort in daily life. If frequent urination or urgency is severe, outdoor activities may be restricted, and if the urine stream is weak and it takes a long time to urinate, it may be stressful in itself.

 

Some people suffer from sleep deprivation due to nocturia, which can make daily life difficult. Because there are so many causes of urination disorders and there are various treatment methods, if you are suffering from urination disorders, it is necessary to receive appropriate examination and treatment through a urology department.
