이번 협약은 통합돌봄 대상자 발굴부터 퇴원환자 단기주거 지원, 식사지원까지 끊김 없이 이어지는 서비스 연계가 핵심이다.

협약에는 남구청을 비롯해 남구종합사회복지관, 대명사회복지관, 남구시니어클럽, 남구지역자활센터, 그리고 지역주민 돌봄활동가 ‘늘봄지기’가 참여한다. 각 기관과 주민이 힘을 모아 원스톱 통합돌봄 체계 구축에 나선 것이다.

특히 눈길을 끄는 건 퇴원환자를 위한 임시주거 공간 '돌봄보금자리'다. 남구청은 LH와 협력해 퇴원 후 거처가 마땅치 않은 주민들에게 임시로 머물 공간을 제공하고, 의료·돌봄 서비스와 연계해 일상 복귀까지 밀착 지원한다. 퇴원 후 곧장 격리되거나 재입원하는 악순환의 고리를 끊겠다는 의지가 담겼다.

돌봄의 첫 단계는 지역 주민 참여로 시작한다. 요양보호사 자격을 가진 시니어 돌봄활동가 ‘늘봄지기’가 대상자 가정을 직접 찾아 돌봄이 필요한 이웃을 발굴하고, 정기적으로 건강과 생활 상태를 점검한다. 이들은 그 누구보다 먼저 위험 신호를 포착하는 촘촘한 사회적 안전망 역할을 맡고 있다.

지역 내 두 복지관은 권역별로 사례관리를 분담한다. 남구종합사회복지관과 대명사회복지관은 의료·주거·돌봄을 잇는 케어매니지먼트를 맡아, 각 대상자에 맞춘 맞춤형 서비스를 설계한다.

식사지원은 남구지역자활센터의 '오레시피' 사업단이 책임진다. 통합돌봄 대상자들은 주 2회, 최대 3개월간 반찬을 지원받으며, 이 과정에서 안부 확인과 고독사 예방까지 이뤄진다. 자활사업과 돌봄이 결합돼 새로운 지역 일자리도 창출될 전망이다.

이번 사업은 2026년 3월 시행 예정인 ‘의료·요양 등 지역돌봄 통합지원법’을 앞둔 선제적 시범 모델이라는 점에서도 의미가 남다르다. 제도 시행에 따른 현장 혼란을 덜고, 실제 효과적인 통합돌봄 구조를 미리 마련하겠다는 취지다.

조재구 남구청장은 “늘봄지기의 촘촘한 사회 연결망과 자활센터의 고독사 예방, 두 복지관의 맞춤형 서비스가 유기적으로 돌아가는 구조를 만들겠다”며, “어르신과 장애인분들이 퇴원 이후에도 지역에서 안심하고 살아갈 수 있도록 실질적인 변화를 이뤄가겠다”고 밝혔다.

Nam-gu Office (Mayor Cho Jae-gu) has signed a public-private partnership agreement to promote the "Our Village Care Management Center," a specialized local project.

This space is linked to medical and care services to provide close support until patients return to their normal lives. This commitment embodies the commitment to breaking the vicious cycle of immediate isolation or readmission after discharge.

The first step in providing care begins with the participation of local residents. Senior care workers, known as "Neulbomjigi," are certified nursing assistants who visit the homes of residents, identify neighbors in need of care, and regularly monitor their health and living conditions. They serve as a tight social safety net, identifying warning signs before anyone else.

Two local welfare centers share case management responsibilities by region. Namgu Community Welfare Center and Daemyung Community Welfare Center are responsible for care management, linking medical care, housing, and care, designing customized services for each individual.

Meal support is provided by the "O'Recipe" project team at the Namgu Community Self-Reliance Center. Recipients of integrated care receive side dishes twice a week for up to three months, ensuring their well-being and preventing lonely deaths. The combination of self-reliance and care is expected to create new local jobs.

This project is also significant as a preemptive pilot model ahead of the "Integrated Support for Regional Care, including Medical and Nursing Care," scheduled to take effect in March 2026. The goal is to alleviate the confusion on-site caused by the implementation of the system and establish an effective integrated care structure in advance.

Nam-gu District Mayor Cho Jae-gu stated, "We will create a structure where Neulbomjigi's dense social network, the Self-Reliance Center's prevention of lonely deaths, and the two welfare centers' customized services work together organically. We will achieve tangible changes so that seniors and people with disabilities can live in peace in their communities even after discharge."
