【한국건강관리협회 】홍은희 인천시지부 원장=우리나라 전체 사망의 1/4을 차지할 정도로 질병 부담이 큰 심혈관 질환. 촌각을 다투는 질환인만큼 경각심을 갖고 생활하는 것이 무엇보다 중요하다. 그렇다면 심혈관 질환을 예방하고 관리하는 방법에는 무엇이 있을까?
뇌졸중·심근경색 초기 증상 숙지하고 발생 즉시 병원으로
심뇌혈관 질환이란 심근경색 · 협심증 등 심장질환, 뇌졸중 등 뇌혈관질환, 고혈압 · 당뇨병 · 고지혈증 · 동맥경화증 등의 합병질환을 일컫는다. 우리나라 전체 사망의 1/4을 차지할 정도로 질병 부담이 크고 발생 시 사망이나 심각한 장애를 초래할 수 있어 주의가 필요하다.
특히 추운 날씨가 지속되는 겨울철에는 혈관이 수축되고 혈압이 상승하기 때문에 심뇌혈관 질환에 따른 돌연사 발생률이 2배로 늘어난다. 그중 주요 사망 원인은 심근경색과 뇌졸중이다. 심뇌혈관 질환은 발병 시 초기 대처가 중요한데 심근경색은 2시간, 뇌졸중은 3시간 내에 치료를 받아야 한다.
심근경색은 심장근육에 혈액을 공급하는 관상동맥이 막혀 사망에 이르게 하고 뇌졸중은 뇌에 혈액을 공급하는 혈관이 막히거나 터져서 사망에 이르거나 뇌손상으로 인한 신체장애가 나타나게 하는 질환이다.
일상생활 시 갑작스러운 가슴 통증이 30분 이상 지속되거나 호흡곤란, 식은땀, 구토, 현기증 등의 증상이 나타난다면 심근경색을 의심해보아야 한다.
또한 한쪽 마비, 갑작스러운 언어 및 시각장애, 어지럼증, 심한 두통을 호소하는 경우 뇌졸중의 초기 증상으로 의심하고, 최대한 빨리 119 연락 또는 택시 등을 이용하여 가깝고 큰 병원 응급실로 가서 재관류 요법(막힌 혈관을 다시 흐르게 뚫어주는 것)을 받으면 발생하기 전과 같은 정상수준이나 장애를 거의 의식하지 않을 수 있는 상태까지 호전될 수 있다.
하지만 아직까지 심뇌혈관 질환에 대한 경각심이 부족해 증상 시작 후 병원 도착까지의 시간이 지연되고 있으며, 후유증으로 인한 사망과 재발률도 상당하다. 증상이 나타나면 팔다리를 주물러줘야 하며, 바늘로 손발 끝을 따거나 의식이 혼미한 환자에게 물이나 약을 먹이려는 등의 행동은 상태를 더 악화시킬 수 있으니 피해야 한다.
심뇌혈관 질환 예방·관리 생활수칙 준수
겨울철 심뇌혈관 질환은 특히 노인, 당뇨, 고혈압 환자에게 더 위험할 수 있다. 그러나 심혈관질환이 중장년층뿐만 아니라 건강한 젊은 층에서도 가족력이나 기름진 음식의 섭취, 잦은 음주와 흡연 등의 잘못된 생활습관으로 발병률이 높기 때문에 각별한 주의가 필요하다.
갑자기 추위에 노출되지 않도록 목도리, 장갑 등을 착용해 보온에 각별히 신경 쓰고 본인의 혈압, 혈당, 콜레스테롤 수치를 알고 관리하며 건강한 생활습관을 실천하는 등 평소에 심뇌혈관 질환 예방관리수칙을 지켜야 한다.
우선 담배만 끊어도 심뇌혈관 질환의 위험성을 크게 줄일 수 있다. 실제로 금연 후 1년이 지나면 심근경색 및 뇌졸중에 걸릴 위험이 절반으로 줄어든다.
또한 연말 송년회 등 술자리에서 폭음을 조심해야 한다. 한국인의 음주율은 매년 꾸준히 증가하고 고위험 음주자 비율이 늘어나고 있다. 한두 잔의 술은 혈관이 좋아지거나 막혀서 생기는 허혈성 심뇌혈관 질환 예방에 도움을 줄 수도 있지만, 과도한 음주는 부정맥과 심근병증을 유발하고 뇌졸중 위험을 증가시킨다. 실제로 OECD에서는 과도한 음주로 뇌졸중을 일으키는 위험성이 평소보다 2.7배정도 높은 것으로 발표했다.
만병의 근원인 스트레스도 심뇌혈관 질환의 주원인이다. 스트레스는 혈압을 높이고 부정맥을 유발한다. 동맥경화를 촉진해 심뇌혈관 질환의 위험요인이 되는 것이다.
평소 음식을 짜게 먹는 습관이 있으면 싱겁게 골고루 먹는 습관을 기르고, 채소와 생선을 충분히 섭취해야 한다. 가능한 한 매일 30분 이상 적절한 운동을 해줘야하며, 적정 체중과 허리둘레를 유지해줘야 한다. 또한 스트레스는 혈압을 상승시키고 부정맥을 유발하며 동맥경화를 촉진하므로 긍정적인 마인드도 심혈관 질환 예방에 중요한 역할을 한다.
고혈압, 당뇨병, 이상지질혈증(고지혈증)은 심혈관 질환의 중요 원인으로 꾸준히 치료해야 한다. 이 질환들은 합병증이 생기기 전까지 뚜렷한 증상이 없기 때문에 고위험군 또는 만성질환자가 아니더라도 40대 이후에는 정기적으로 혈압, 혈당, 콜레스테롤을 측정해야 한다. 그리고 특히 고위험군은 뇌졸중과 심근경색의 응급증상을 꼭 숙지하고 증상 발생 즉시 병원에 갈 수 있도록 대비해야 한다.
