스트레스가 파킨슨병의 원인이 된다

 미국 로널드 레이건대통령, 전설의 복서 무하마드 알리 등이 투병한 것으로 유명한 ‘파킨슨병’은 3대 퇴행성 뇌질환으로 꼽힌다. 파킨슨병의 가장 중요한 발병 인자는 ‘고령’이며 만성적 스트레스는 파킨슨병의 발병률을 높인다고 한다.

 

파킨슨병은 알츠하이머 치매에 뒤이어 신경퇴행성 질환 중 두 번째로 흔한 질병이다. 평균 발병 연령은 55세로, 세계적으로 보았을 때 60세 이상의 경우 1%가 파킨슨병을 앓고 있다. 운동 능력 저하와 더불어 자율신경계 이상 증상을 동반하는 파킨슨병은 40~50세의 생산인구에서의 발병률이 치매보다도 9배 높은 것으로 알려져있다.

 

파킨슨병을 말할 때 빠지지 않는 것은 인구 고령화로 인한 꾸준한 증가 추세이다. 치매, 파킨슨병과 같은 노년기에 주로 발생하는 퇴행성 질환에 대한 관심도 고령화와 함께 늘어나고 있다.

 

시니어(senior)는 ‘(계급지위)가 고위의, (스포츠에서 상급 수준에 이른)성인을 위한, 연장자’의 뜻이다. 흔히 우리는 경험이 많은, 상위 직책의 사람이나 나이가 많은 사람을 표현할 때 사용한다. 최근에는 일자리가 부족해지고 개인주의적 성향이 보편화되며, 사회가 분열과 혐오의 양상을 띠면서 시니어는 존경, 배움, 보호의 대상이기보다는 도전과 비판의 대상이 되고 있다.

 

그들의 스트레스는 안팎으로 많아진다. 수명이 연장되면서 시니어로서 살아가는 기간이 점점 늘어나고 있고 스트레스에 노출되는 기간도 길어졌다. 이러한 ‘시니어 스트레스’가 파킨슨병과 관련이 있을까?

 

◇ 파킨슨병은 무엇인가

 

파킨슨병은 도파민이라는 신경전달 물질이 부족해지면서 몸동작이 느려지는 서동증과 더불어 떨림, 강직, 보행 장애로 나타난다. 도파민은 인체 운동 능력을 조절하는 역할을 하는데, 이러한 도파민을 생산하는 신경세포가 퇴행하고 사멸하면서 운동 능력이 떨어지면 파킨슨병으로 진단한다.

 

피킨슨병이라고 하면 손 떨림이 가장 먼저 떠오르지만 모든 떨림이 파킨슨병을 의미하는 것은 아니다. 파킨슨병에 특징적인 떨림은 가만히 있을 때만 손, 발이 떨리고 젓가락질을 할 때처럼 운동 시에는 문제가 없다. 또한 한쪽에서 시작하여 병이 깊어지면서 반대쪽으로 퍼지게 된다. 떨림 외에도 몸이 굳고 움직임이 느려지며, 균형 감각이 떨어지는 것이 대표적인 증상이다.

 

떨림 증상이 없는 파킨슨병도 적지 않기 때문에 다양한 증상을 관찰해서 조기 발견해야 한다. 대개 도파민 분비 신경세포가 70%까지 없어지고 나서야 증상이 발생하는 것으로 알려져 있어 운동 증상이 나타난 시점에는 이미 퇴행성 변화가 진행된 상태이다. 이러한 운동 증상이 나타나기 전에 만성 변비, 기립성 어지러움, 소변 장애, 심한 잠꼬대, 냄새와 맛 구분이 어려워지는 것과 같은 비운동 증상이 먼저 나타난다. 비운동 증상만 있는 시기에 조기 발견하면 좋은 치료 효과를 기대할 수 있다.

 

◇ 스트레스가 파킨슨병의 원인일까

 

스트레스와 파킨슨병의 관계는 이미 100여 년 전에도 장기간의 불안과 정서적 충격은 파킨슨병의 흔한 전조라는 기록이 남아있을 정도로 오래전부터 언급되었다. 실제로 홀로코스트나 포로 생활과 같은 극심한 심리적 스트레스를 받은 사람들에서 파킨슨병의 발병률이 높게 나타났다. 정서적인 스트레스가 상대적으로 감수성이 높은 환자에서 파킨슨병의 원인이 되는 흑질 선조체 변성을 유발할 수 있다고 알려진다.

 

만성적인 스트레스는 시상하부-뇌하수체-부신 축을 장기간 활성화하여 부하를 유발하고 산화 스트레스를 일으켜 도파민 활동 감소로 이어져 파킨슨병을 일으킬 수 있다. 만성 스트레스 요인이 사라지면서 약물치료를 중단할 수 있었던 환자에 대한 보고도 있는 만큼 장기간의 스트레스가 파킨슨증과 유사한 증상을 유발할 수 있다.

 

◇ 파킨슨병의 예방과 증상 완화를 위한 방법

 

세계 파킨슨병 학회에서 말하는 스트레스에 대처하는 방법은 긍정적인 태도와 웃음, 건강한 식단과 영양 섭취, 운동 및 여가, 약물치료가 있다. 웃음은 정신을 고양하는 데 도움이 되는 화학 물질은 방출하고 긴장을 줄여준다. 건강한 식단은 신경독성을 줄여줄 뿐만 아니라 장 건강이 뇌와 긴밀하게 연관되어 있는 것으로 알려져 신경계 질환 예방에 도움을 준다.

 

특히 견과류와 토마토, 당근, 파프리카, 브로콜리 등 색깔이 분명한 채소들이 뇌 신경세포 보호에 도움이 된다. 운동은 스트레스를 줄여줄 뿐만 아니라 신경 가소성(신경세포 회로 구축 및 재구성)을 향상시켜 퇴행성 질환의 예방과 증상 완화에 도움을 준다. 감당할 수 없는 스트레스는 약물치료의 도움을 고려해보는 것이 좋다.

 

파킨슨병은 다양한 형태로 나타나기 때문에 신경과 전문의의 신경 진찰과 비운동 증상에 대한 체계적인 설문이 필수적이다. 비운동 증상에는 후각, 어지러움, 배뇨 장애, 기억력 저하, 기분장애 등의 신체 여러 부위에 다양한 증상이 있어, 신체 기능 전반에 대한 검사가 필요하다.

 

파킨슨병의 치료는 부족한 도파민을 보충하는 것을 기본으로 도파민 보충에는 다양한 약제가 개발되어 있어 상황에 맞게 전문의와 상의하여 복용해야 한다. 파킨슨병은 운동·비운동 증상이 서서히 악화되면서 일상생활 능력에 큰 지장을 준다. 예방과 완치가 아직까지는 어렵지만 증상을 완화시키고 경과를 늦출 수 있는 치료가 있는 만큼 적절한 검사를 통한 조기 진단과 치료가 중요하다.
